Mechanism of Action
Cefuroxime is a second-generation cephalosporin that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs). Clavulanic acid is a beta-lactamase inhibitor that protects cefuroxime from degradation by beta-lactamase enzymes produced by resistant bacteria, thereby extending its antibacterial spectrum.

Absorption
Cefuroxime axetil is absorbed from the gastrointestinal tract and rapidly hydrolyzed to cefuroxime. Absorption is increased with food. Clavulanic acid is also well absorbed.
Metabolism
Cefuroxime is not metabolized significantly. Clavulanic acid is extensively metabolized.
Side Effects
Contraindications
- Hypersensitivity to cefuroxime, clavulanic acid, any other cephalosporin antibiotic, or any penicillin antibiotic.
- History of cholestatic jaundice/hepatic dysfunction associated with penicillin or cephalosporin use.
Drug Interactions
Warfarin
May potentiate anticoagulant effect.
Probenecid
Increases and prolongs serum concentrations of cefuroxime.
Oral Contraceptives
May reduce efficacy of oral contraceptives (advise additional contraceptive methods).
Antacids/H2 blockers
May decrease bioavailability of cefuroxime axetil.
Storage
Store dry powder below 30°C. Store reconstituted suspension in a refrigerator (2-8°C) and discard after 7-10 days.
Overdose
Symptoms may include gastrointestinal upset, convulsions. Treatment is supportive; hemodialysis can be used to remove cefuroxime and clavulanate from the blood.
