Overview of the medicine
Cerox-CV 500 mg Tablet is a combination antibiotic containing Cefuroxime Axetil and Clavulanic Acid. It is used to treat a wide range of bacterial infections including respiratory tract infections, urinary tract infections, skin and soft tissue infections, and otitis media. Cefuroxime is a cephalosporin antibiotic that kills bacteria by inhibiting cell wall synthesis, while Clavulanic Acid prevents the inactivation of cefuroxime by beta-lactamase enzymes produced by some bacteria, thereby extending its spectrum of activity.
Uses & Indications
Dosage
Adults
One tablet (Cefuroxime 500 mg + Clavulanic acid 125 mg) twice daily for 7-10 days, or as directed by the physician.
Elderly
No specific dose adjustment is usually required unless there is significant renal impairment.
Renal_impairment
Dose adjustment may be necessary based on creatinine clearance. For CrCl 10-30 mL/min, one tablet once daily. For CrCl <10 mL/min, one tablet every 48 hours.
How to Take
Should be taken orally with food to enhance absorption and reduce gastrointestinal discomfort.
Mechanism of Action
Cefuroxime is a second-generation cephalosporin that binds to penicillin-binding proteins (PBPs) in the bacterial cell wall, thereby inhibiting bacterial cell wall synthesis. Clavulanic acid is a beta-lactamase inhibitor that irreversibly binds to and inactivates beta-lactamase enzymes, protecting cefuroxime from degradation and allowing it to exert its bactericidal effect against beta-lactamase-producing strains.

Excretion
Mainly excreted unchanged by the kidneys via glomerular filtration and tubular secretion. Clavulanic acid is also primarily excreted by the kidneys.
Half life
Approximately 1-1.5 hours for cefuroxime; 1 hour for clavulanic acid.
Absorption
Well absorbed from the gastrointestinal tract after oral administration, especially with food. Cefuroxime axetil is rapidly hydrolyzed to cefuroxime in the intestinal mucosa and blood.
Metabolism
Cefuroxime is not significantly metabolized; clavulanic acid is extensively metabolized.
Side Effects
Contraindications
- Known hypersensitivity to cefuroxime, clavulanic acid, any other cephalosporin, or penicillin
- History of cholestatic jaundice/hepatic dysfunction associated with penicillin or cephalosporin use
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin. INR monitoring is recommended.
Probenecid
Increases cefuroxime plasma levels and prolongs its half-life by reducing renal tubular secretion.
Oral Contraceptives
May reduce the efficacy of oral contraceptives. Advise additional contraceptive methods.
Antacids/H2 blockers
May reduce the bioavailability of cefuroxime axetil if taken concurrently; administer at least 1-2 hours apart.

Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
