Description
Overview of the medicine
Cefuroxime Axetil is a second-generation cephalosporin antibiotic used to treat a wide range of bacterial infections, including respiratory tract, urinary tract, skin and soft tissue, ear, nose, and throat infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
250 mg twice daily for most infections. For more severe infections or certain types like lower respiratory tract infections, 500 mg twice daily may be prescribed. For uncomplicated urinary tract infections, 125 mg twice daily. For uncomplicated gonorrhoea, a single 1 g dose.
Elderly
Similar to adult dosage, but dose adjustment may be necessary in case of significantly impaired renal function.
Renal_impairment
Dose adjustment is required based on creatinine clearance. For creatinine clearance 10-29 mL/min, 125-250 mg once daily. For <10 mL/min or hemodialysis patients, 125-250 mg every 48 hours or after dialysis.
How to Take
Take orally, preferably with food, to enhance absorption and minimize gastrointestinal discomfort. Do not crush or chew the tablet.
Mechanism of Action
Cefuroxime Axetil is hydrolyzed to cefuroxime in the body. Cefuroxime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), thus exhibiting bactericidal activity against susceptible organisms.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved approximately 2-3 hours after oral administration with food.
Excretion
Excreted unchanged in the urine, primarily by glomerular filtration and tubular secretion.
Half life
Approximately 1-1.5 hours in healthy adults.
Absorption
Well absorbed from the gastrointestinal tract and rapidly hydrolyzed by esterases to cefuroxime. Absorption is enhanced when taken with food.
Metabolism
Cefuroxime itself is not metabolized in the body. Cefuroxime axetil is hydrolyzed to the active drug cefuroxime.
Side Effects
Contraindications
- Hypersensitivity to cefuroxime or any other cephalosporin antibiotic
- History of severe hypersensitivity reaction to any other type of beta-lactam antibacterial agents (e.g., penicillins)
Drug Interactions
Probenecid
Concomitant administration with probenecid increases cefuroxime AUC and peak plasma levels, and prolongs its elimination half-life.
Oral Contraceptives
May reduce the efficacy of oral contraceptives; advise using additional barrier contraception.
Antacids/H2-receptor antagonists
Drugs that reduce gastric acidity (e.g., antacids, H2-receptor antagonists) may decrease the bioavailability of cefuroxime axetil.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include convulsions, encephalopathy, and other neurological sequelae. Treatment is largely symptomatic and supportive. Hemodialysis may be useful in removing the drug from the blood.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of impaired fertility or harm to the fetus. However, use in pregnancy should only be considered if the benefits outweigh the risks. Cefuroxime is excreted in small amounts in breast milk; caution should be exercised when administered to a nursing mother.
