Overview of the medicine
Ciprofloxacin 500 mg Tablet is a fluoroquinolone antibiotic used to treat a variety of bacterial infections, including urinary tract infections, respiratory tract infections, skin and soft tissue infections, bone and joint infections, and certain sexually transmitted diseases.
Uses & Indications
Dosage
Adults
Typical dosage is 250-750 mg twice daily, depending on the infection type and severity.
Elderly
Dosage adjustment may be necessary in elderly patients with impaired renal function. Usually, adult doses apply, but monitor renal function.
Renal_impairment
Dosage reduction is required for creatinine clearance less than 50 mL/min. For CrCl 30-50 mL/min, 250-500 mg every 12 hours. For CrCl <30 mL/min, 250-500 mg every 18-24 hours.
How to Take
Take orally with a glass of water, with or without food. Avoid taking with dairy products, antacids containing aluminum or magnesium, or iron/zinc supplements, as these can interfere with absorption. Maintain adequate hydration.
Mechanism of Action
Ciprofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes essential for bacterial DNA replication, transcription, repair, and recombination. This leads to bacterial cell death.

Onset
Usually within 1-2 hours.
Excretion
Primarily excreted renally (50-70% as unchanged drug) and to a lesser extent via the fecal route.
Half life
Approximately 3-5 hours.
Absorption
Rapidly and well absorbed from the gastrointestinal tract; bioavailability is approximately 70-80%.
Metabolism
Partially metabolized in the liver to four active metabolites (desethyleneciprofloxacin, sulfociprofloxacin, oxociprofloxacin, and formylciprofloxacin).
Side Effects
Contraindications
- Hypersensitivity to ciprofloxacin or other fluoroquinolones
- Concomitant administration with tizanidine
- History of tendinitis or tendon rupture associated with quinolone use
Drug Interactions
Warfarin
Potentiates anticoagulant effect, monitor INR.
Tizanidine
Concomitant use is contraindicated due to increased tizanidine plasma concentrations, leading to hypotension and somnolence.
Methotrexate
Increased plasma levels of methotrexate, leading to toxicity.
Theophylline
Increased plasma levels of theophylline, leading to toxicity.
Corticosteroids
Increased risk of tendon rupture.
Antacids (Mg/Al), Iron/Zinc supplements
Reduced ciprofloxacin absorption; administer 2 hours before or 6 hours after.

Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Limited data available. Symptoms may include dizziness, tremor, headache, confusion, abdominal pain, renal and hepatic impairment, and crystalluria. Treatment is symptomatic and supportive; ensure adequate hydration, gastric lavage, and activated charcoal if appropriate.

Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. Ciprofloxacin is excreted in breast milk; therefore, exercise caution when administering to a nursing woman or advise to discontinue breastfeeding.
