Description
Overview of the medicine
Ciprofloxacin is a fluoroquinolone antibiotic used to treat various bacterial infections. It works by stopping the growth of bacteria. This injection formulation is used for severe infections or when oral administration is not feasible.
Uses & Indications
Dosage
Adults
Usually 200-400 mg by slow intravenous infusion (over 60 minutes) every 8-12 hours, depending on the severity and type of infection. Max dose up to 1.2 g/day.
Elderly
Dosage adjustment may be required based on renal function.
Renal_impairment
Reduced dosage is necessary. For CrCl 30-60 mL/min, 200-400 mg every 12 hours. For CrCl <30 mL/min, 200-400 mg every 18-24 hours or half the usual dose.
How to Take
Civox-200 mg injection should be administered by slow intravenous infusion over a period of 60 minutes. It should not be given by rapid intravenous bolus injection. Ensure appropriate dilution if necessary and check for particulate matter before administration.
Mechanism of Action
Ciprofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, essential enzymes involved in DNA replication, transcription, repair, and recombination. This leads to bacterial cell death.

Rapid (within minutes for IV infusion).
Excretion
Primarily renal (approximately 50-70% as unchanged drug), with some fecal excretion.
Half life
3 to 5 hours (may be prolonged in renal impairment).

Intravenously administered Ciprofloxacin is 100% bioavailable.
Metabolism
Partially metabolized in the liver (via CYP1A2) to less active metabolites.
Side Effects
Contraindications
- •Hypersensitivity to Ciprofloxacin, any other fluoroquinolone, or any component of the formulation.
- •Concomitant administration with Tizanidine due to the risk of increased Tizanidine plasma concentrations, which can lead to clinically significant hypotension and sedation.
Drug Interactions
Phenytoin
May alter Phenytoin levels; monitoring is advised.
Tizanidine
Concomitant use is contraindicated due to a significant increase in Tizanidine plasma concentrations, leading to hypotension and sedation.
Methotrexate
May increase Methotrexate plasma concentrations, increasing the risk of toxicity.
Theophylline
Ciprofloxacin can increase plasma levels of Theophylline, leading to increased risk of toxicity. Monitor Theophylline levels closely.
Corticosteroids
Increased risk of tendon rupture with concomitant use.
Warfarin and other oral anticoagulants
Ciprofloxacin may enhance the anticoagulant effect of Warfarin and other oral anticoagulants. Monitor INR/PT closely.
Storage
Store below 25°C. Protect from light. Do not freeze. Keep out of reach of children.
Overdose
In the event of an acute overdose, reversible renal toxicity has been reported in some cases. Management involves symptomatic and supportive care. Hemodialysis or peritoneal dialysis removes only a small amount of Ciprofloxacin from the body.
Pregnancy & Lactation
Pregnancy Category C. Ciprofloxacin should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Ciprofloxacin is excreted into human breast milk. Due to the potential for serious adverse reactions in nursing infants, a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
