Description
Overview of the medicine
Deflazacort is a synthetic glucocorticoid with anti-inflammatory and immunosuppressive properties. It is used to treat various inflammatory and autoimmune conditions, including Duchenne muscular dystrophy.
Uses & Indications
Dosage
Adults
Dosage varies greatly depending on the condition. For Duchenne muscular dystrophy, typical dosage is 0.9 to 1.5 mg/kg once daily. For other conditions, 6-18 mg once daily or as prescribed by a physician.
Elderly
No specific dose adjustment is generally required, but caution is advised due to increased susceptibility to side effects. Lower effective doses should be considered.
Renal_impairment
No dose adjustment is usually required for mild to moderate renal impairment. Caution is advised in severe renal impairment.
How to Take
Deflazit tablet should be taken orally, with or without food. It can be taken once daily. Do not crush or chew the tablet.
Mechanism of Action
Deflazacort acts by binding to specific glucocorticoid receptors within target cells, leading to changes in gene expression. This modulates the synthesis of proteins involved in inflammatory and immune responses, thereby exerting its anti-inflammatory and immunosuppressive effects.
Pharmacokinetics
Onset
Pharmacological effects typically begin within hours.
Excretion
Excreted mainly via urine (approximately 70%) and feces (approximately 30%).
Half life
The active metabolite has a plasma half-life of approximately 1.2 to 1.7 hours.
Absorption
Rapidly absorbed after oral administration and undergoes rapid conversion to its active metabolite, 21-desacetyl deflazacort.
Metabolism
Primarily metabolized in the liver to its active metabolite, 21-desacetyl deflazacort, which is responsible for most of its pharmacological activity.
Side Effects
Contraindications
- Systemic fungal infections
- Hypersensitivity to deflazacort or any components of the formulation
- Administration of live or live-attenuated vaccines during treatment
Drug Interactions
Warfarin
Deflazacort may alter the anticoagulant effect of warfarin; close monitoring of INR is recommended.
Ketoconazole, Itraconazole
May increase plasma concentrations of deflazacort; dosage adjustment may be needed.
Rifampicin, Phenytoin, Carbamazepine
May decrease plasma concentrations of deflazacort; dosage adjustment may be needed.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Acute overdose is rare. Symptoms may include exaggerated side effects like fluid retention, hypertension, and hyperglycemia. Management involves symptomatic and supportive treatment, including electrolyte balance correction and withdrawal of the drug.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in breast milk; breastfeeding is not recommended during treatment. Consult a doctor.
