Description
Overview of the medicine
Deflazacort is a corticosteroid used to treat a wide range of inflammatory and autoimmune conditions. It helps to reduce inflammation and suppress the immune system.
Uses & Indications
Dosage
Adults
Initial dose typically 6-18 mg once daily, adjusted according to the patient's response and tolerability. Maintenance dose usually 3-18 mg daily.
Elderly
Lower doses may be required, and patients should be monitored for side effects.
Renal_impairment
No specific dose adjustment is generally needed for renal impairment, but caution is advised in severe cases.
How to Take
Take orally, with or without food. It can be taken once daily.
Mechanism of Action
Deflazacort binds to glucocorticoid receptors in the cytoplasm, forming a complex that translocates to the nucleus. This complex modulates gene expression, leading to the synthesis of anti-inflammatory proteins and inhibition of pro-inflammatory mediators, thereby exerting its anti-inflammatory and immunosuppressive effects.
Pharmacokinetics
Onset
Effects may be seen within hours to days depending on the condition being treated.
Excretion
Primarily excreted in the urine (approximately 70%) and faeces (approximately 30%) as metabolites.
Half life
The active metabolite (21-desacetyl deflazacort) has a plasma half-life of 1.5-2 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract; peak plasma concentrations of the active metabolite (21-desacetyl deflazacort) occur within 1.5-2 hours.
Metabolism
Undergoes rapid first-pass metabolism in the liver to its active metabolite, 21-desacetyl deflazacort.
Side Effects
Contraindications
- Systemic fungal infections
- Known hypersensitivity to deflazacort or any component of the formulation
- Administration of live or live attenuated vaccines in patients receiving immunosuppressive doses of deflazacort
Drug Interactions
NSAIDs
Increased risk of gastrointestinal ulceration.
Warfarin
Deflazacort may modify the anticoagulant effect of warfarin; close monitoring of INR is required.
Ketoconazole, Erythromycin
May increase deflazacort levels by inhibiting liver enzymes.
Phenobarbital, Phenytoin, Rifampicin
May decrease deflazacort levels by inducing liver enzymes.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Acute overdose is rare. Management involves symptomatic and supportive treatment. Long-term overdose may lead to corticosteroid side effects.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Small amounts may pass into breast milk; caution is advised during breastfeeding.

Doctor Notes
- Emphasize the importance of gradual tapering of the dose to prevent adrenal crisis.
- Advise patients about potential for increased susceptibility to infections and to report fever or other signs of infection promptly.
- Regular monitoring of blood pressure, blood glucose, and bone density is crucial for long-term therapy.
- Consider calcium and vitamin D supplementation to mitigate bone-related side effects.
Patient Guidelines
- Do not stop taking deflazacort abruptly; it must be tapered off gradually under medical supervision to avoid withdrawal symptoms.
- Inform your doctor about all other medications, including over-the-counter drugs and herbal supplements.
