Description
Overview of the medicine
Denixil 2 mg Tablet is a benzodiazepine derivative primarily used for the short-term treatment of anxiety disorders, insomnia, and as a muscle relaxant. It works by affecting brain chemicals that may be unbalanced in people with anxiety.
Uses & Indications
Dosage
Adults
For anxiety: 2 mg, 1-2 times daily. Max 6 mg/day. For insomnia: 2 mg at bedtime. For muscle spasms: 2 mg, 2-3 times daily.
Elderly
Initiate with a lower dose, e.g., 1 mg daily, and adjust gradually based on response and tolerability. Max 3 mg/day.
Renal_impairment
Dose adjustment may be necessary in patients with severe renal impairment. Start with a reduced dose and monitor carefully.
How to Take
Denixil tablet should be taken orally, with or without food. Swallow the tablet whole with water. Do not crush, chew, or break it.
Mechanism of Action
Denixil binds to specific benzodiazepine receptors on the gamma-aminobutyric acid (GABA) A receptor complex in the central nervous system. This binding enhances the inhibitory effects of GABA, leading to increased chloride ion influx, hyperpolarization of the neuron, and decreased neuronal excitability.
Pharmacokinetics
Onset
Anxiolytic effects typically begin within 30-60 minutes.
Excretion
Mainly excreted in the urine as metabolites, with a small amount excreted in feces.
Half life
Elimination half-life ranges from 10 to 20 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations reached within 1-2 hours.
Metabolism
Primarily metabolized in the liver via oxidative pathways (e.g., CYP3A4, CYP2C19) to active and inactive metabolites.
Side Effects
Contraindications
- •Known hypersensitivity to benzodiazepines or any component of the formulation
- •Myasthenia gravis
- •Severe respiratory insufficiency
- •Sleep apnea syndrome
- •Severe hepatic insufficiency
- •Acute narrow-angle glaucoma
Drug Interactions
Cimetidine and Erythromycin
Can inhibit hepatic metabolism of Denixil, potentially increasing its plasma concentration and enhancing sedative effects.
Antifungals (e.g., ketoconazole) and Protease inhibitors
May inhibit metabolism of Denixil, leading to increased plasma levels and effects.
Alcohol and other CNS depressants (e.g., opioids, antihistamines, sedatives)
Increased risk of sedation, respiratory depression, and hypotension. Avoid concomitant use.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, confusion, impaired coordination, slurred speech, and coma. Management involves general supportive measures, maintaining a clear airway, and monitoring vital signs. Flumazenil, a benzodiazepine receptor antagonist, may be used as an antidote, but should be used with caution due to the risk of precipitating withdrawal symptoms or seizures.
Pregnancy & Lactation
Pregnancy Category D. Denixil is not recommended during pregnancy, especially during the first trimester, due to potential risk of congenital malformations. It passes into breast milk and may cause sedation in the infant; therefore, use is not recommended during lactation.
