Overview of the medicine
Dexifen 400 mg Tablet is a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain and inflammation in conditions like osteoarthritis, rheumatoid arthritis, dysmenorrhea, and dental pain. It works by blocking the production of certain natural substances in the body that cause inflammation and pain.
Uses & Indications
Dosage
Adults
Usually 200 mg to 400 mg two to three times daily. Max 1200 mg/day. For chronic conditions, 400 mg once or twice daily.
Elderly
Start with the lowest effective dose and monitor for adverse effects. Dose adjustment might be necessary based on renal function.
Renal_impairment
Use with caution, reduced dosage may be necessary in severe renal impairment. Avoid if GFR < 30 mL/min.
How to Take
Take orally with water, preferably with or after food to minimize gastrointestinal discomfort. Do not crush or chew the tablet.
Mechanism of Action
Dexibuprofen is the pharmacologically active S(+)-enantiomer of ibuprofen. It inhibits prostaglandin synthesis by reversibly blocking cyclooxygenase (COX-1 and COX-2) enzymes, leading to reduced inflammation, pain, and fever.

Excreted mainly in the urine (approximately 90%) as metabolites and their conjugates, with a small amount excreted in feces.

Rapidly and almost completely absorbed from the gastrointestinal tract. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Primarily metabolized in the liver by oxidation and conjugation with glucuronic acid.

- Third trimester of pregnancy
- Cerebrovascular or other active bleeding

Symptoms include nausea, vomiting, abdominal pain, drowsiness, dizziness, and headache. More severe cases may involve gastrointestinal bleeding, acute renal failure, hepatic dysfunction, hypotension, and respiratory depression. Management is supportive and symptomatic; gastric lavage or activated charcoal may be considered if ingested recently.
Pregnancy & Lactation
Avoid during the third trimester of pregnancy due to the risk of premature closure of the ductus arteriosus and renal dysfunction in the fetus. Use during the first and second trimesters only if clearly necessary and potential benefits outweigh risks. Small amounts may pass into breast milk; generally considered compatible with breastfeeding at low doses, but use with caution.
