Overview of the medicine
Ema 40 mg capsule contains Esomeprazole, a proton pump inhibitor that reduces the amount of acid produced in the stomach. It is used to treat conditions caused by too much stomach acid.
Uses & Indications
Dosage
Adults
Typical dose is 20 mg or 40 mg once daily, depending on the condition. For erosive esophagitis, 20 mg or 40 mg once daily for 4 to 8 weeks. For GERD symptoms, 20 mg once daily for 4 weeks.
Elderly
No dosage adjustment is generally required, but caution is advised in patients with severe hepatic impairment.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment.
How to Take
Take the capsule whole with a glass of water, at least one hour before a meal. Do not chew or crush the capsule.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that specifically blocks the H+/K+-ATPase enzyme system (proton pump) in the gastric parietal cells, thereby inhibiting the final step of acid secretion.

Onset
Acid suppression begins within 1 hour, maximal effect within a few days of regular dosing.
Excretion
Mainly excreted in urine (approximately 80%) and feces (approximately 20%) as metabolites.
Half life
Approximately 1.3 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 1-2 hours.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 enzyme system (CYP2C19 and CYP3A4).
Side Effects
Contraindications
- Known hypersensitivity to Esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Warfarin
Increased INR/prothrombin time; monitor closely.
Tacrolimus
Increased tacrolimus levels; monitor blood concentrations.
Clopidogrel
Reduced effectiveness of clopidogrel; consider alternative antiplatelet therapy.
Methotrexate
Increased methotrexate levels; monitor for toxicity.
Ketoconazole/Itraconazole
Reduced absorption due to increased gastric pH.
Storage

Overdose
Experience with Esomeprazole overdose is very limited. Management should be symptomatic and supportive. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. It is not known whether Esomeprazole is excreted in human milk; use with caution in nursing mothers.
