Description
Overview of the medicine
Tranexamic acid is an antifibrinolytic agent that prevents the breakdown of blood clots, used to control excessive bleeding in various conditions.
Uses & Indications
Dosage
Adults
Typical IV dose is 500-1000 mg (1-2 ampoules) 2-3 times daily, administered slowly over 5-10 minutes. Doses vary significantly based on indication and patient weight.
Elderly
Dose adjustment may be needed based on renal function. Use with caution.
Renal_impairment
Dose adjustment is required; significantly reduced doses are used in patients with severe renal insufficiency.
How to Take
For intravenous administration only. Administer slowly (over 5-10 minutes) or by infusion to avoid hypotension.
Mechanism of Action
Tranexamic acid inhibits fibrinolysis by reversibly binding to plasminogen, preventing its activation to plasmin, thereby stabilizing the fibrin clot and reducing bleeding.
Pharmacokinetics
Onset
Rapid, typically within minutes of intravenous administration.
Excretion
Primarily excreted unchanged in the urine via glomerular filtration, with about 95% excreted within 24 hours.
Half life
Plasma half-life is approximately 2-11 hours, with an elimination half-life of about 3 hours.
Absorption
Rapidly absorbed after intravenous administration, achieving peak plasma levels immediately.
Metabolism
Minimally metabolized; less than 5% of the dose undergoes metabolism.

Contraindications
- •Active intravascular clotting (e.g., deep vein thrombosis, pulmonary embolism)
- •Severe renal impairment (risk of accumulation)
- •History of convulsions (especially in subarachnoid hemorrhage)
- •Acquired defective color vision (cannot monitor for visual toxicity)
Drug Interactions
Thrombolytic agents
Antagonistic effect, reduces the efficacy of thrombolytics.
Oral Contraceptives/Estrogens

Factor IX complex concentrates/anti-inhibitor coagulant concentrates
Increased risk of thrombosis.
Storage
Store below 30°C. Protect from light. Do not freeze. Keep out of reach of children.
