Overview of the medicine
Esogel 20 mg capsule contains Esomeprazole, a proton pump inhibitor, used to reduce stomach acid production. It is prescribed for conditions like GERD, ulcers, and heartburn.
Uses & Indications
Dosage
Adults
GERD: 20 mg or 40 mg once daily for 4-8 weeks. Erosive Esophagitis: 40 mg once daily for 4-8 weeks. NSAID-associated ulcers: 20 mg once daily for 4-8 weeks. H. pylori eradication: 20 mg twice daily for 7-10 days (with antibiotics).
Elderly
No dose adjustment generally required for elderly patients.
Renal_impairment
No dose adjustment generally required for patients with renal impairment.
How to Take
Take one capsule whole with water at least one hour before a meal. Do not chew or crush the capsule. For patients who cannot swallow, the capsule can be opened and the pellets mixed with a small amount of water or soft food and consumed immediately.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that works by irreversibly binding to and inhibiting the H+/K+-ATPase (proton pump) in the gastric parietal cells. This action effectively blocks the final step in gastric acid secretion, leading to a reduction in stomach acid.
Pharmacokinetics
Onset
Acid-inhibitory effect usually begins within 1 hour of administration.
Excretion
Approximately 80% of metabolites are excreted in the urine and the remainder in the feces.
Half life
Plasma elimination half-life is approximately 1.0-1.5 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations occurring within 1-2 hours. Bioavailability is approximately 64% after a single 20 mg dose, increasing to 89% with repeated once-daily dosing.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to Esomeprazole, substituted benzimidazoles, or any component of the formulation.
- •Concomitant use with Nelfinavir (due to significant reduction in Nelfinavir plasma levels).
Drug Interactions
Diazepam
Decreased clearance of diazepam (CYP2C19 inhibition).
Warfarin
Increased INR and bleeding risk (monitor INR).
Clopidogrel
Reduced effectiveness of clopidogrel (CYP2C19 inhibition).
Methotrexate
Possible increased methotrexate levels (consider temporary PPI withdrawal).
Atazanavir/Nelfinavir
Reduced plasma concentrations of these antivirals.
Ketoconazole/Itraconazole
Reduced absorption due to increased gastric pH.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Experience with Esomeprazole overdose is limited. Symptoms may include gastrointestinal upset, drowsiness, tachycardia. Treatment is symptomatic and supportive. Esomeprazole is not readily dialyzable.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but adequate human studies are lacking. Use during pregnancy only if clearly needed. Excreted in breast milk, caution is advised during breastfeeding.
