Description
Overview of the medicine
Esomeprazole is a proton pump inhibitor that reduces stomach acid production. It is used to treat gastroesophageal reflux disease (GERD), peptic ulcers, and other conditions caused by excessive stomach acid.
Uses & Indications
Dosage
Adults
20-40 mg once daily, depending on the indication. For GERD/Erosive Esophagitis: 20-40 mg once daily for 4-8 weeks. For H. pylori eradication: 40 mg once daily for 10 days (with appropriate antibiotics).
Elderly
No specific dose adjustment is generally required for elderly patients.
Renal_impairment
No dose adjustment is necessary for patients with mild to moderate renal impairment. Caution is advised in severe renal impairment.
How to Take
Take orally, usually 30-60 minutes before a meal. Swallow the capsule whole with water; do not chew or crush.
Mechanism of Action
Esomeprazole selectively and irreversibly inhibits the H+/K+-ATPase (proton pump) in the gastric parietal cells, thereby reducing gastric acid secretion.
Pharmacokinetics
Onset
Within 1 hour.
Excretion
Primarily excreted renally (approximately 80%) and in feces (approximately 20%).
Half life
Approximately 1-1.5 hours.
Absorption
Rapidly absorbed after oral administration, but absorption may be delayed by food. Bioavailability increases with repeated dosing.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 system (CYP2C19 and CYP3A4).
Side Effects
Contraindications
- •Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- •Concomitant use with nelfinavir.
Drug Interactions
Warfarin
May increase INR and prothrombin time, increasing risk of bleeding.
Tacrolimus
May increase tacrolimus serum levels.
Clopidogrel
May reduce the antiplatelet effect of clopidogrel.
Methotrexate
May increase serum levels and prolong the elimination of methotrexate.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are generally mild and may include confusion, blurred vision, tachycardia, nausea, sweating, headache, and dry mouth. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed. Caution is advised during lactation as it is unknown if esomeprazole is excreted in human milk.
