Description
Overview of the medicine
Esomeprazole is a proton pump inhibitor (PPI) that decreases the amount of acid produced in the stomach. It is used to treat conditions such as gastroesophageal reflux disease (GERD) and ulcers.
Uses & Indications
Dosage
Adults
For erosive esophagitis: 20 or 40 mg once daily for 4 to 8 weeks. For GERD symptoms: 20 mg once daily for 4 weeks. For H. pylori eradication: 20 mg twice daily for 10 days (in combination therapy).
Elderly
No dosage adjustment is generally required in elderly patients.
Renal_impairment
No dosage adjustment is generally required in patients with renal impairment. However, caution is advised in severe cases.
How to Take
Esogel-20-mg-tablet should be taken at least one hour before a meal. Swallow the tablet whole; do not chew or crush it. For patients who have difficulty swallowing, the tablet can be dispersed in a glass of non-carbonated water and taken immediately.
Mechanism of Action
Esomeprazole works by irreversibly blocking the H+/K+-ATPase (proton pump) in the gastric parietal cells. This action effectively reduces both basal and stimulated gastric acid secretion, leading to an increase in gastric pH.
Pharmacokinetics
Onset
Onset of acid suppression within 1 hour.
Excretion
Approximately 80% is excreted in the urine as metabolites, and the remaining 20% in the feces.
Half life
Approximately 1-1.5 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations occurring within 1-2 hours. Bioavailability is approximately 64% with a single 40 mg dose and increases to 89% with repeated once-daily administration.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, mainly by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- •Concomitant use with nelfinavir.
Drug Interactions
Diazepam
May increase plasma concentrations of diazepam due to inhibition of CYP2C19.
Warfarin
Increased INR and prothrombin time, monitor for bleeding. Adjust warfarin dose as needed.
Clopidogrel
Esomeprazole may reduce the antiplatelet effect of clopidogrel. Concomitant use should be avoided.
Methotrexate
Increased methotrexate levels, risk of toxicity. Consider temporary esomeprazole discontinuation or lower methotrexate dose.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are generally mild and include drowsiness, confusion, blurred vision, tachycardia, nausea, sweating, flushing, headache, and dry mouth. Treatment should be symptomatic and supportive. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown whether esomeprazole is excreted in human milk, so caution should be exercised when administered to a nursing mother.
