Description
Overview of the medicine
Esomeprazole is a proton pump inhibitor (PPI) that reduces the amount of acid produced in the stomach. It is used to treat conditions caused by too much stomach acid.
Uses & Indications
Dosage
Adults
GERD: 20-40 mg once daily for 4-8 weeks. Erosive Esophagitis: 20-40 mg once daily for 4-8 weeks. H. pylori eradication: 20 mg twice daily with antibiotics for 7-10 days.
Elderly
No dosage adjustment generally required.
Renal_impairment
No dosage adjustment required.
How to Take
Take one capsule whole with water at least one hour before a meal. Do not crush, chew, or open the capsule.
Mechanism of Action
Esomeprazole is the S-isomer of omeprazole. It acts by specifically inhibiting the H+/K+-ATPase (proton pump) in the gastric parietal cells, thereby blocking the final step in gastric acid secretion.

Onset
Acid secretion inhibition occurs within 1 hour.
Excretion
Primarily excreted via urine (80%) and feces (20%).
Half life
Approximately 1.5 hours.
Absorption
Rapidly absorbed after oral administration. Bioavailability is approximately 64% (20 mg) and 89% (40 mg) after repeated once-daily dosing.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) system, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Diazepam
Decreased clearance of diazepam.
Warfarin
Increased INR (International Normalized Ratio), monitor coagulation.
Clopidogrel
May reduce the antiplatelet effect of clopidogrel, concurrent use not recommended.
Methotrexate
Increased methotrexate levels, monitor for toxicity.
Ketoconazole, Itraconazole
May decrease absorption due to increased gastric pH.
