Overview of the medicine
Esomiloc 40 mg Capsule contains Esomeprazole, a proton pump inhibitor (PPI) that reduces the amount of acid produced in the stomach. It is used to treat conditions such as gastroesophageal reflux disease (GERD), stomach ulcers, and other conditions caused by too much stomach acid.
Uses & Indications
Dosage
Adults
For GERD: 20-40 mg once daily for 4-8 weeks. For Erosive Esophagitis: 20-40 mg once daily for 4-8 weeks. For H. pylori eradication: 40 mg once daily for 10 days (in combination therapy).
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment is necessary for patients with impaired renal function.
How to Take
Take the capsule whole, at least one hour before a meal. Do not chew or crush the capsule. If swallowing is difficult, the capsule can be opened, and the pellets mixed with a small amount of water or soft food and swallowed immediately.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that works by irreversibly binding to the H+/K+-ATPase enzyme system (the proton pump) in the gastric parietal cells. This action inhibits the final step in acid production, thereby reducing gastric acid secretion.
Pharmacokinetics
Onset
Onset of acid suppression within 1 hour.
Excretion
Excreted primarily through the kidneys (approximately 80%) and via the feces (approximately 20%).
Half life
Approximately 1-1.5 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma levels achieved within 1-2 hours. Bioavailability increases with repeated dosing.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 system (CYP2C19 and CYP3A4).
Side Effects
Contraindications
- Known hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Warfarin
Increased INR and prothrombin time have been reported, monitor for bleeding.
Nelfinavir
Decreases plasma levels of nelfinavir, concomitant use is contraindicated.
Tacrolimus
Increased serum tacrolimus levels have been reported.
Clopidogrel
Esomeprazole may reduce the antiplatelet effect of clopidogrel by inhibiting CYP2C19.
Methotrexate
May increase serum methotrexate levels, monitor for toxicity.

Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Limited experience with overdose. Symptoms may include gastrointestinal disturbances and fatigue. Management should be symptomatic and supportive. Esomeprazole is not readily dialyzable.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. It is unknown whether esomeprazole is excreted in human milk, use with caution during lactation.
