Description
Overview of the medicine
Esomeprazole is a proton pump inhibitor that reduces the amount of acid produced in the stomach. It is used to treat conditions such as gastroesophageal reflux disease (GERD), ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
Usually 20 mg or 40 mg once daily, depending on the indication. For GERD: 20-40 mg once daily for 4-8 weeks. For H. pylori: 20 mg twice daily for 10-14 days with antibiotics.
Elderly
No specific dosage adjustment is generally required for elderly patients, but caution is advised in those with severe hepatic impairment.
Renal_impairment
No dosage adjustment is required for patients with renal impairment.
How to Take
Take orally, at least one hour before a meal. Swallow the tablet whole; do not chew or crush it. If swallowing is difficult, the tablet can be dispersed in half a glass of non-carbonated water.
Mechanism of Action
Esomeprazole works by irreversibly blocking the H+/K+-ATPase pump (proton pump) located in the gastric parietal cells. This inhibition prevents the final step of acid production, thereby reducing both basal and stimulated gastric acid secretion.
Pharmacokinetics
Onset
Acid suppression starts within 1 hour.
Excretion
Metabolites are primarily excreted in the urine (approx 80%) and feces (approx 20%).
Half life
Approximately 1-1.5 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations reached within 1-2 hours. Bioavailability is approximately 64% after a single dose and increases to 89% with repeated daily dosing.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 system, mainly by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Warfarin
Monitor INR and prothrombin time; increased risk of bleeding.
Nelfinavir
Concomitant use is contraindicated due to significantly decreased nelfinavir plasma levels.
Clopidogrel
May reduce the antiplatelet effect of clopidogrel; consider alternative antiplatelet therapy.
Methotrexate
Increased and prolonged serum levels of methotrexate, especially with high doses.
Diazepam, Citalopram, Phenytoin
Increased plasma concentrations of these drugs due to inhibition of CYP2C19.
Ketoconazole, Itraconazole, Erlotinib
Reduced absorption due to increased gastric pH.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
There is limited experience with esomeprazole overdose. Symptoms may include gastrointestinal disturbances and weakness. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether esomeprazole is excreted in human milk; use with caution during lactation.
