Exephin 1 gm Injection contains Cefepime, a fourth-generation cephalosporin antibiotic used to treat various severe bacterial infections.
Uses & Indications
Dosage
Adults
Typical adult dosage is 1-2 gm intravenously (IV) every 8-12 hours, depending on the type and severity of infection.
Elderly
Dosage adjustment may be necessary in elderly patients with impaired renal function, based on creatinine clearance.
Renal_impairment
Dosage must be adjusted significantly in patients with renal impairment (creatinine clearance < 60 mL/min) to prevent accumulation and potential CNS toxicity.
How to Take
Exephin 1 gm Injection is administered intravenously (IV) over 30 minutes, or as a deep intramuscular (IM) injection. The powder must be reconstituted with an appropriate diluent (e.g., Sterile Water for Injection, 0.9% Sodium Chloride Injection).
Mechanism of Action
Cefepime exerts its bactericidal effect by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs), leading to the disruption of cell wall integrity and ultimately bacterial cell lysis.

Side Effects
Contraindications
- •Known hypersensitivity to cefepime, any other cephalosporin antibiotic, penicillins, or any other beta-lactam antibiotic.
- •Patients with a history of severe immediate hypersensitivity reactions to beta-lactam antibiotics.
Drug Interactions
Aminoglycosides
Concurrent use may increase the risk of nephrotoxicity and ototoxicity. Monitor renal function closely.
Loop Diuretics (e.g., Furosemide)
May increase the risk of nephrotoxicity when used with cephalosporins.
Oral Anticoagulants (e.g., Warfarin)
May enhance the anticoagulant effect by altering gut flora; monitor INR.
Storage
Store the dry powder at controlled room temperature (below 25°C) and protect from light. Reconstituted solutions should be used immediately or stored for a specified duration (e.g., 24 hours at room temperature or 7 days under refrigeration) as per manufacturer's instructions.
Overdose
Symptoms of overdose may include encephalopathy, seizures, neuromuscular excitability. Management is symptomatic and supportive. Hemodialysis may be useful in facilitating the removal of cefepime from the body, especially in patients with impaired renal function.
