Exephin 500 mg Injection contains Ceftriaxone, a third-generation cephalosporin antibiotic, used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
Typical adult dose is 1-2 g intravenously or intramuscularly once daily, or in two divided doses. Max dose 4 g/day.
Elderly
No specific dose adjustment typically required, but caution in renal/hepatic impairment.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment; for severe impairment (CrCl <10 mL/min), maximum 2g/day.
How to Take
For intravenous use, reconstitute with appropriate diluent and administer slowly over 30 minutes. For intramuscular use, reconstitute with Lidocaine 1% and administer deeply into a large muscle.
Mechanism of Action
Ceftriaxone acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, thereby inhibiting the third and final stage of bacterial cell wall synthesis. This leads to the lysis of bacterial cells.

Excretion
About 50-60% excreted unchanged in urine, and 40-50% excreted via bile and feces.

Approximately 6-9 hours.
Absorption
Rapid and complete absorption after intramuscular administration; IV administration provides immediate bioavailability.
Metabolism
Partially metabolized in the liver to inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to ceftriaxone, any other cephalosporin, or penicillin
- •Neonates with hyperbilirubinemia
- •Concurrent use with calcium-containing intravenous solutions in neonates (≤28 days old)
Drug Interactions
Aminoglycosides
Synergistic antibacterial effect but increased risk of nephrotoxicity (rare).
Calcium-containing solutions
Fatal precipitation risk in neonates. Avoid concomitant administration.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding due to vitamin K antagonism.
Loop diuretics (e.g., Furosemide)
May increase ceftriaxone serum levels.
Storage
Store below 30°C, protected from light and moisture. Reconstituted solutions should be used immediately or stored as per manufacturer's guidelines (e.g., up to 24 hours at 2-8°C).
Overdose
Symptoms may include nausea, vomiting, diarrhea, and convulsions in severe cases. Management is primarily supportive; hemodialysis or peritoneal dialysis is generally not effective.
