Overview of the medicine
Exephin 2gm Injection contains Cefepime, a fourth-generation cephalosporin antibiotic with a broad spectrum of activity against gram-positive and gram-negative bacteria, including Pseudomonas aeruginosa. It is used to treat serious bacterial infections.
Uses & Indications
Dosage
Adults
1-2 gm intravenously (IV) or intramuscularly (IM) every 8-12 hours, depending on the type and severity of infection.
Elderly
No specific dosage adjustment for elderly patients with normal renal function; however, renal function should be assessed and dosage adjusted if impaired.
Renal_impairment
Dosage must be adjusted based on creatinine clearance to prevent accumulation.
How to Take
Administered by deep intramuscular injection or by intravenous infusion over 30 minutes. Reconstitute the powder with an appropriate diluent (e.g., Sterile Water for Injection, 0.9% Sodium Chloride Injection).
Mechanism of Action
Cefepime is a bactericidal antibiotic that acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs), disrupting peptidoglycan synthesis, leading to bacterial cell lysis and death.

Onset
Rapid, typically within minutes for IV administration.
Excretion
Primarily renal excretion, with about 85% of an administered dose recovered unchanged in the urine.
Half life
Approximately 2-2.2 hours in individuals with normal renal function.
Absorption
Rapid and complete absorption after intravenous or intramuscular administration. Peak plasma concentrations are achieved shortly after IV infusion.
Metabolism
Minimally metabolized; approximately 15% is hydrolyzed to N-methylpyrrolidine (NMP).
Side Effects
Contraindications
- •Known hypersensitivity to cefepime, any other cephalosporin, penicillins, or any other beta-lactam antibiotic.
- •History of severe hypersensitivity reaction to any component of the formulation.
Drug Interactions
Loop Diuretics
May increase serum concentrations of cefepime.
Aminoglycosides
Increased risk of nephrotoxicity and ototoxicity.
Oral Anticoagulants (e.g., Warfarin)
May increase the anticoagulant effect.
Storage
Store below 25-30°C. Protect from light. Keep out of the reach of children. Do not freeze the powder.
Overdose
In case of overdose, symptoms may include encephalopathy (e.g., convulsions, confusion, stupor), myoclonus, and neuromuscular excitability. Treatment should be symptomatic and supportive. Hemodialysis can remove cefepime from the body.
Pregnancy & Lactation
**Pregnancy Category B:** Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. **Lactation:** Cefepime is excreted in human milk in very low concentrations. Caution should be exercised when administered to a nursing woman.
