Overview of the medicine
Flucloxacillin is a narrow-spectrum beta-lactam antibiotic of the penicillin class. It is used to treat bacterial infections caused by penicillinase-producing staphylococci, such as skin and soft tissue infections, bone infections, endocarditis, and septicemia. The 125 mg suspension is typically prescribed for pediatric use.
Uses & Indications
Dosage
Adults
Not typically prescribed in 125 mg suspension for adults. Adult dosage for flucloxacillin usually 250-500 mg every 6 hours, orally.
Elderly
No specific dosage adjustment for elderly with normal renal function. Monitor renal function.
Renal_impairment
Dosage reduction may be necessary for severe renal impairment (creatinine clearance < 10 ml/min).
How to Take
Oral administration. For optimal absorption, take 30-60 minutes before meals, on an empty stomach. Shake the suspension well before each use. Use a measuring spoon or cup for accurate dosage.
Mechanism of Action
Flucloxacillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death. It is resistant to degradation by beta-lactamase enzymes produced by some bacteria.

Excretion
Primarily excreted unchanged by the kidneys (glomerular filtration and tubular secretion); also excreted in bile.

Metabolism
Partially metabolized in the liver to inactive metabolites.
Side Effects
Contraindications
- •Known hypersensitivity to flucloxacillin, other penicillins, or any component of the formulation.
- •History of flucloxacillin-associated jaundice or hepatic dysfunction.
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin. Monitor INR.
Probenecid
Increases and prolongs plasma levels of flucloxacillin.
Methotrexate
May increase methotrexate toxicity by reducing its renal clearance.
Tetracyclines
May antagonize the bactericidal effect of flucloxacillin.
Storage
Store dry powder below 25°C in a dry place, protected from light and moisture. Keep out of reach of children. Store reconstituted suspension in a refrigerator (2-8°C). Do not freeze.
Overdose
Symptoms of overdose may include nausea, vomiting, diarrhea, and electrolyte imbalance. Treatment is symptomatic and supportive. Hemodialysis may be used to remove flucloxacillin from circulation in severe cases.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. Use during pregnancy only if clearly needed. Flucloxacillin is excreted in breast milk; caution advised during lactation.
