Overview of the medicine
Flucloxacillin is a narrow-spectrum beta-lactam antibiotic of the penicillin class. It is used to treat infections caused by Gram-positive bacteria susceptible to its action, particularly Staphylococcus aureus.
Uses & Indications
Dosage
Adults
Typically 250-500 mg every 6 hours, given by slow intravenous injection or infusion, or intramuscular injection. Higher doses (up to 1g every 4-6 hours) may be used for severe infections.
Elderly
No specific dose adjustment for elderly with normal renal function, but caution is advised in those with impaired renal function.
Renal_impairment
Reduce dose in severe renal impairment (e.g., creatinine clearance <10 mL/min). Max dose 1g every 8-12 hours.
How to Take
Administered intravenously (slow injection or infusion) or intramuscularly. Reconstitute the powder with sterile water for injection according to manufacturer's instructions before use.
Mechanism of Action
Flucloxacillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death. It is resistant to degradation by beta-lactamase enzymes produced by some bacteria.

Rapid, within minutes of IV administration; within 30-60 minutes of IM administration.
Excretion
Mainly excreted unchanged in urine via renal tubular secretion and glomerular filtration. Small amount excreted in bile.

Absorption
Rapidly absorbed after intramuscular administration, peak plasma levels typically within 30-60 minutes. Bioavailability is good.
Metabolism
Limited hepatic metabolism (primarily to inactive metabolites).
Side Effects
Contraindications

Drug Interactions
Warfarin
May alter anticoagulant effect; close monitoring of INR is recommended.
Probenecid
Increases and prolongs plasma levels of flucloxacillin by competing for renal tubular secretion.
Methotrexate
May increase methotrexate toxicity due to reduced renal clearance.
Live bacterial vaccines (e.g., Typhoid vaccine)
May reduce the therapeutic effect of the vaccine.
Storage
Store below 25°C in a dry place, protected from light. Keep out of reach of children. Reconstituted solution should be used immediately or stored for a short period as per manufacturer's guidelines.
Overdose
Symptoms: Nausea, vomiting, diarrhea, electrolyte disturbances. In severe cases, neurotoxicity (e.g., convulsions) may occur, especially in patients with impaired renal function. Management: Symptomatic and supportive treatment. Flucloxacillin is hemodialyzable.
Pregnancy & Lactation
Pregnancy: Category B. Generally considered safe, but use only if clearly needed. Lactation: Excreted in breast milk in small amounts; generally considered safe, but monitor infant for potential adverse effects like diarrhea or rash.
