Overview of the medicine
Flucloxacillin is a penicillin antibiotic used to treat a wide range of bacterial infections, especially those caused by penicillinase-producing staphylococci. It is particularly effective against skin and soft tissue infections, bone infections, and respiratory tract infections.
Uses & Indications
Dosage
Adults
Usual adult dose is 250-500 mg every 6 hours, preferably administered 1 hour before meals. For severe infections, the dose may be increased up to 1 g every 6 hours.
Elderly
Similar to adult dosage, but caution and dose reduction may be necessary in severe renal impairment.
Renal_impairment
In severe renal impairment (creatinine clearance < 10 mL/min), a dose reduction or increased dosing interval (e.g., 250 mg every 8-12 hours) is recommended to avoid accumulation.
How to Take
Take the capsule orally, preferably on an empty stomach, at least 30-60 minutes before food or 2 hours after food, with a glass of water. This helps ensure optimal absorption.
Mechanism of Action
Flucloxacillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs) located inside the bacterial cell wall. This leads to the disruption of the cell wall structure, resulting in bacterial cell lysis and death.
Pharmacokinetics
Onset
Within 30-60 minutes after oral administration.
Excretion
Primarily excreted unchanged in the urine (about 65-70% of a dose) through glomerular filtration and tubular secretion. A small amount is excreted in bile.
Half life
Approximately 0.5 to 1.5 hours in individuals with normal renal function.
Absorption
Well absorbed orally, though absorption is reduced by food. Peak plasma concentrations are achieved in approximately 1-2 hours.
Metabolism
Partially metabolized in the liver to inactive metabolites.
Side Effects
Contraindications
- •Known hypersensitivity to flucloxacillin, other penicillins, or any beta-lactam antibiotics.
- •History of flucloxacillin-associated jaundice or hepatic dysfunction.
- •Severe liver disease.
Drug Interactions
Warfarin
Flucloxacillin may increase the anticoagulant effect of warfarin, requiring close monitoring of INR and possible dose adjustment of warfarin.
Probenecid
Concomitant administration of probenecid delays the renal excretion of flucloxacillin, leading to increased and prolonged plasma concentrations.
Methotrexate
May increase methotrexate toxicity by reducing its renal clearance.
Oral Contraceptives
The efficacy of oral contraceptives may be reduced, although this interaction is less commonly reported with flucloxacillin compared to broad-spectrum antibiotics.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptoms may include nausea, vomiting, diarrhea, and electrolyte imbalance. Treatment should be symptomatic and supportive. Gastric lavage or activated charcoal may be considered if ingestion is recent. Hemodialysis is not highly effective in removing flucloxacillin.
Pregnancy & Lactation
Pregnancy Category B. Flucloxacillin is generally considered safe for use during pregnancy, but it should be used only if clearly needed. It is excreted in small amounts into breast milk; monitor breastfed infants for potential gastrointestinal disturbances or allergic reactions.
