Overview of the medicine
Flucloxacillin is an antibiotic belonging to the penicillin group, effective against a wide range of bacterial infections. It is particularly active against beta-lactamase-producing Staphylococci and Streptococci, making it suitable for treating skin and soft tissue infections, bone and joint infections, and certain respiratory tract infections.
Uses & Indications
Dosage
Adults
Oral: 250-500 mg every 6 hours (four times daily). For severe infections, dosage may be increased up to 1g every 6 hours. Administer 30-60 minutes before meals for optimal absorption.
Elderly
No specific dosage adjustment required unless there is significant renal impairment. Monitor renal function.
Renal_impairment
CrCl 10-50 mL/min: 250 mg every 8-12 hours. CrCl <10 mL/min: 250 mg every 12-24 hours. Severe impairment may require further reduction.
How to Take
Flucloxacillin capsules should be taken orally with water, preferably on an empty stomach (30-60 minutes before meals or at least 2 hours after meals) to ensure maximum absorption. Do not chew or crush the capsule; swallow it whole.
Mechanism of Action
Flucloxacillin is a bactericidal antibiotic that acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs), which are essential for cell wall formation, leading to bacterial cell lysis and death. It is resistant to degradation by beta-lactamase enzymes produced by some bacteria.

Excretion
Primarily excreted by the kidneys (renal excretion), with 65-80% of the dose recovered unchanged in the urine within 6 hours.
Half life
Approximately 0.5-1 hour in individuals with normal renal function.
Absorption
Well absorbed from the gastrointestinal tract, especially when taken on an empty stomach. Peak plasma concentrations are usually achieved within 1-2 hours after oral administration.
Metabolism
Partially metabolized in the liver to inactive metabolites. About 10-20% undergoes metabolic conversion.
Side Effects
Contraindications
- •Hypersensitivity to flucloxacillin, other penicillins, or any component of the formulation.
- •History of flucloxacillin-associated jaundice or hepatic dysfunction.
Drug Interactions
Warfarin
Concurrent use may alter the international normalized ratio (INR), requiring close monitoring and possible dose adjustment of warfarin.
Probenecid
May increase and prolong flucloxacillin plasma concentrations by reducing its renal tubular secretion.
Methotrexate
Flucloxacillin may reduce renal excretion of methotrexate, potentially increasing its toxicity.
Oral Contraceptives
May rarely reduce the efficacy of oral contraceptives; advise additional non-hormonal contraception.
Storage
Store below 30°C in a cool, dry place, protected from light and moisture. Keep out of sight and reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, diarrhea, and electrolyte imbalance. Management is primarily symptomatic and supportive. Hemodialysis is not effective in removing flucloxacillin from the body.
Pregnancy & Lactation
Pregnancy Category B. Flucloxacillin is generally considered safe for use during pregnancy, though it should be used with caution and only if clearly needed. It is excreted in small amounts into breast milk; breastfeeding is generally considered compatible, but monitor the infant for potential adverse effects such as diarrhea, candidiasis, or skin rash.
