Overview of the medicine
Fuclav 125 mg Suspension is an antibiotic medication used to treat a wide range of bacterial infections. It contains Cefuroxime, a second-generation cephalosporin antibiotic, and Clavulanic Acid, a beta-lactamase inhibitor, which prevents bacterial enzymes from inactivating Cefuroxime.
Uses & Indications
Dosage
Adults
Not typically recommended for adults as 125mg suspension. Higher strengths (e.g., 250mg, 500mg tablets) are used for adults.
Elderly
No specific dose adjustment needed unless severe renal impairment.
Renal_impairment
Dose adjustment required. For CrCl 10-29 mL/min, administer standard dose every 24 hours. For CrCl <10 mL/min, administer standard dose every 48 hours.
How to Take
Take orally, preferably with food to enhance absorption and reduce gastrointestinal discomfort. Shake the bottle well before each use. Use the provided measuring spoon or cup for accurate dosing.
Mechanism of Action
Cefuroxime is a bactericidal antibiotic that acts by inhibiting bacterial cell wall synthesis. It has affinity for penicillin-binding proteins (PBPs). Clavulanic acid is a beta-lactamase inhibitor that irreversibly binds to and inactivates beta-lactamase enzymes, thereby protecting cefuroxime from degradation.

Excretion
Mainly excreted unchanged by the kidneys (glomerular filtration and tubular secretion) for cefuroxime. Clavulanic acid is primarily eliminated via renal excretion and metabolism.
Half life
Approximately 1-1.5 hours for both cefuroxime and clavulanic acid.
Absorption
Cefuroxime axetil is well absorbed from the gastrointestinal tract and rapidly hydrolyzed to cefuroxime. Absorption is enhanced with food. Clavulanic acid is also well absorbed.
Metabolism
Cefuroxime is not metabolized; cefuroxime axetil is hydrolyzed in the intestinal mucosa and blood to release cefuroxime. Clavulanic acid is extensively metabolized.
Side Effects
Contraindications
- •Hypersensitivity to Cefuroxime, Clavulanic Acid, or any cephalosporin antibiotic.
- •History of severe hypersensitivity reaction (e.g., anaphylaxis) to any other beta-lactam antibiotic (e.g., penicillins, carbapenems).
Drug Interactions
Probenecid
Increases cefuroxime plasma levels and prolongs its half-life by reducing renal tubular secretion.
Oral Anticoagulants
May potentiate the anticoagulant effect of warfarin and other oral anticoagulants; monitor INR.
Oral Contraceptives
May reduce the effectiveness of oral contraceptives; advise additional contraceptive methods.
Antacids/H2 blockers/PPIs
May reduce the bioavailability of cefuroxime axetil by increasing gastric pH. Administer at least 1 hour before or 2 hours after antacids.
Storage
Store dry powder below 30°C in a dry place, away from light and moisture. After reconstitution, store in a refrigerator (2-8°C) and do not freeze.
Overdose
Symptoms of overdose include gastrointestinal disturbances (nausea, vomiting, diarrhea) and convulsions. Treatment is symptomatic and supportive; hemodialysis may be considered in severe cases to remove cefuroxime and clavulanic acid from circulation.
Pregnancy & Lactation
Pregnancy Category B. Use with caution during pregnancy if clearly needed. Both cefuroxime and clavulanic acid are excreted in breast milk; use with caution in breastfeeding mothers.
