Overview of the medicine
Fuclav 250 mg Tablet is a combination antibiotic used to treat a wide range of bacterial infections. It contains Amoxicillin, a penicillin-type antibiotic, and Clavulanic Acid, a beta-lactamase inhibitor that prevents bacteria from destroying Amoxicillin.
Uses & Indications
Dosage
Adults
For adults and children weighing 40 kg or more: One Fuclav 250 mg / 125 mg tablet three times daily. Doses may be increased in severe infections as per physician's advice.
Elderly
No specific dose adjustment required for elderly patients unless there is evidence of renal impairment.
Renal_impairment
Dosage should be adjusted based on the patient's creatinine clearance. Reduced doses are necessary for severe renal impairment.
How to Take
Take the tablet whole with a glass of water, preferably at the start of a meal to minimize potential gastrointestinal intolerance. Do not chew or crush the tablet.
Mechanism of Action
Amoxicillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death. Clavulanic Acid inactivates various beta-lactamase enzymes produced by bacteria, extending Amoxicillin's spectrum of activity to include many beta-lactamase-producing strains that would otherwise be resistant.
Pharmacokinetics
Onset
The onset of action is generally rapid, with clinical improvement observed within a few days of starting treatment.
Excretion
Both components are primarily excreted unchanged by the kidneys, through glomerular filtration and tubular secretion. A smaller proportion is eliminated via bile and feces.
Half life
The elimination half-life is approximately 1 to 1.5 hours for Amoxicillin and about 1 hour for Clavulanic Acid.
Absorption
Both Amoxicillin and Clavulanic Acid are well absorbed from the gastrointestinal tract after oral administration. Peak plasma concentrations are typically reached within 1-2 hours.
Metabolism
Amoxicillin is partially metabolized; about 10-25% is converted to inactive penicilloic acid. Clavulanic Acid is extensively metabolized.
Side Effects
Contraindications
- •Hypersensitivity to Amoxicillin, Clavulanic Acid, any penicillin, or any other component of the formulation.
- •History of severe hypersensitivity reaction to any other beta-lactam agent (e.g., cephalosporins, carbapenems, monobactams).
- •History of cholestatic jaundice/hepatic dysfunction associated with Amoxicillin/Clavulanic Acid.
Drug Interactions
Probenecid
Decreased renal tubular secretion of Amoxicillin, leading to increased and prolonged blood levels of Amoxicillin.
Allopurinol
Increased incidence of skin rash when co-administered with Amoxicillin.
Methotrexate
Reduced excretion of methotrexate, potentially increasing toxicity.
Oral Anticoagulants (e.g., Warfarin)
Increased prothrombin time/INR. Close monitoring is advised.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include gastrointestinal symptoms (e.g., nausea, vomiting, diarrhea) and disturbances of fluid and electrolyte balance. In isolated cases, crystalluria leading to renal failure has been observed. Management is symptomatic and supportive, focusing on fluid and electrolyte balance. Hemodialysis can be used to remove the drug from circulation.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. However, use during pregnancy should be considered only if clearly needed. Both Amoxicillin and Clavulanic Acid are excreted into breast milk; therefore, caution should be exercised when administered to a nursing woman.
