Description
Overview of the medicine
Fuclav-500 mg Tablet is an antibiotic containing Amoxicillin and Clavulanic Acid. It is used to treat a wide range of bacterial infections, including respiratory tract infections, urinary tract infections, skin and soft tissue infections, and ear infections. Clavulanic acid helps Amoxicillin work against bacteria that produce beta-lactamase enzymes.
Uses & Indications
Dosage
Adults
For mild to moderate infections: 500 mg/125 mg tablet every 8 hours. For severe infections: 875 mg/125 mg tablet every 12 hours. Duration depends on the infection severity.
Elderly
Dosage adjustment may be necessary in elderly patients with impaired renal function. Otherwise, adult dosage applies.
Renal_impairment
Creatinine clearance 10-30 mL/min: 500 mg/125 mg every 12 hours. Creatinine clearance < 10 mL/min: 500 mg/125 mg every 24 hours. Hemodialysis patients: 500 mg/125 mg every 24 hours, plus an additional dose during and at the end of dialysis.
How to Take
Take orally at the start of a meal to minimize potential gastrointestinal intolerance. Swallow the tablet whole with a glass of water. Do not crush or chew.
Mechanism of Action
Amoxicillin, a beta-lactam antibiotic, inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs). Clavulanic acid is a beta-lactamase inhibitor that protects Amoxicillin from degradation by beta-lactamase enzymes produced by resistant bacteria, thereby extending Amoxicillin's antibacterial spectrum.

Excretion
Primarily excreted unchanged by the kidneys (glomerular filtration and tubular secretion). Small amounts are excreted in feces.
Half life
Approximately 1 to 1.5 hours for both components.
Absorption
Both Amoxicillin and Clavulanic Acid are well absorbed from the gastrointestinal tract after oral administration, with peak plasma concentrations occurring approximately 1-2 hours after dosing. Food has minimal effect on absorption.
Metabolism
Amoxicillin is minimally metabolized. Clavulanic acid is extensively metabolized.
Side Effects
Contraindications
- •Hypersensitivity to penicillin or other beta-lactam antibiotics.
- •History of Amoxicillin/Clavulanate-associated jaundice or hepatic dysfunction.
Drug Interactions
Probenecid
Decreases renal tubular secretion of Amoxicillin, leading to increased and prolonged blood levels of Amoxicillin. Concomitant use is not recommended.
Allopurinol
Increased incidence of skin rash.
Methotrexate
Increased toxicity of methotrexate due to decreased renal excretion. Monitor methotrexate levels.
Oral Anticoagulants
May prolong prothrombin time (INR). Careful monitoring is required.
Oral Contraceptives
May reduce the efficacy of oral contraceptives. Advise alternative or additional contraceptive methods.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include gastrointestinal symptoms (e.g., abdominal pain, vomiting, diarrhea) and fluid and electrolyte imbalance. In a small number of patients, renal impairment and convulsions may occur. Treatment involves symptomatic and supportive measures. Hemodialysis can be used to remove Amoxicillin/Clavulanate from circulation.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. However, use during pregnancy only if clearly needed. Both Amoxicillin and Clavulanic Acid are excreted in breast milk; use with caution during lactation.
