Description
Overview of the medicine
G-Fix contains Cefixime, a third-generation cephalosporin antibiotic, used to treat a wide range of bacterial infections including those of the respiratory tract, urinary tract, and ear.
Uses & Indications
Dosage
Adults
Adults and children over 10 years or weighing over 50 kg: 200-400 mg orally once daily or 200 mg twice daily. For uncomplicated gonorrhea, a single 400 mg dose is recommended.
Elderly
No specific dose adjustment is needed for elderly patients with normal renal function. Close monitoring of renal function is advised.
Renal_impairment
For creatinine clearance 21-60 mL/min, 300 mg daily; for creatinine clearance <20 mL/min, 200 mg daily. Hemodialysis or peritoneal dialysis patients: 200 mg once daily.
How to Take
G-Fix can be taken with or without food. However, taking it with food may help reduce gastrointestinal upset.
Mechanism of Action
Cefixime exerts its bactericidal effect by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, thereby inhibiting the third and final stage of bacterial cell wall synthesis. This leads to bacterial cell lysis and death.
Pharmacokinetics
Onset
Peak plasma concentrations reached within 2-6 hours after oral administration.
Excretion
Primarily excreted unchanged in urine (about 50%) and bile (about 10%).
Half life
Average plasma half-life is 3-4 hours, allowing for once or twice daily dosing.
Absorption
Approximately 40-50% orally absorbed. Absorption may be slightly delayed by food but overall extent is not significantly affected.
Metabolism
Limited hepatic metabolism. Most of the absorbed drug is excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to Cefixime or any other cephalosporin antibiotics.
- History of severe hypersensitivity reactions to penicillin.
Drug Interactions
Nifedipine
Increased Cefixime bioavailability has been reported.
Probenecid
May increase Cefixime serum levels and prolong its half-life by decreasing renal tubular secretion.
Carbamazepine
Concurrent administration may increase carbamazepine plasma concentrations.
Warfarin and other anticoagulants
May prolong prothrombin time (INR), requiring close monitoring and possible dose adjustment.
Storage
Store in a cool, dry place below 30°C, away from direct sunlight and moisture. Keep out of reach of children. Reconstituted suspension should be stored in a refrigerator (2-8°C) and discarded after 7-14 days, depending on formulation.
Overdose
In case of overdose, gastric lavage may be indicated. There is no specific antidote for Cefixime. Cefixime is not removed in significant quantities from the circulation by hemodialysis or peritoneal dialysis. Treatment should be supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus. However, Cefixime should be used during pregnancy only if clearly needed. Cefixime is excreted in small amounts in human milk, so caution should be exercised when administered to a nursing woman.
