Overview of the medicine
Gabatec 25 mg Capsule contains Gabapentin, an anticonvulsant and neuropathic pain agent. It is primarily used to treat epilepsy (partial seizures) and neuropathic pain, such as postherpetic neuralgia.
Uses & Indications
Dosage
Adults
Initial dose for neuropathic pain is typically 300 mg daily, increasing gradually to 900-3600 mg/day in divided doses. For epilepsy, usually 300 mg three times daily, adjusted based on response. (25 mg is a very low dose, usually used for initial titration or specific patient populations/conditions).
Elderly
Dose adjustment may be required based on renal function. Start with lower doses.
Renal_impairment
Dose must be significantly reduced. For CrCl < 30 mL/min, maximum recommended dose is 300 mg once daily. For CrCl < 15 mL/min, 100-300 mg every other day.
How to Take
Gabatec capsules should be swallowed whole with water, with or without food. It can be taken three times daily, usually at equally spaced intervals.
Mechanism of Action
Gabapentin's precise mechanism is not fully understood, but it is thought to modulate the activity of voltage-gated calcium channels, reducing the release of excitatory neurotransmitters. It does not bind to GABA receptors directly.

Onset
For neuropathic pain, onset of pain relief can be gradual, sometimes taking weeks. For seizures, a therapeutic effect can be seen relatively quickly once stable levels are achieved.
Excretion
Excreted unchanged by the kidneys.
Half life
Approximately 5-7 hours in adults with normal renal function.
Absorption
Oral bioavailability is dose-dependent and saturable, decreasing with increasing dose. Peak plasma concentrations are reached within 2-3 hours.
Metabolism
Not significantly metabolized in humans.

Contraindications
- Hypersensitivity to gabapentin or any component of the formulation
Drug Interactions
Antacids
May decrease gabapentin bioavailability. Take gabapentin at least 2 hours after antacid.
Opioids (e.g., Morphine)
May increase gabapentin levels and increase risk of CNS depression.
CNS Depressants (e.g., Alcohol, Sedatives)
Increased risk of CNS depression.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include dizziness, double vision, slurred speech, drowsiness, and mild diarrhea. Management is supportive care, including gastric lavage and activated charcoal if overdose is recent. Hemodialysis can remove gabapentin.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Gabapentin is excreted in breast milk; caution should be exercised when administered to a nursing woman.
