Description
Overview of the medicine
Gabapentin is an anticonvulsant medicine used to treat partial seizures and neuropathic pain such as postherpetic neuralgia and pain due to diabetic neuropathy.
Uses & Indications
Dosage
Adults
For neuropathic pain: Initially 300 mg once daily on Day 1, 300 mg twice daily on Day 2, and 300 mg three times daily on Day 3. Doses can be further titrated up to 1800-3600 mg/day in divided doses. For partial seizures: Initial dose is 300 mg three times daily, which can be increased based on patient response up to 1800-3600 mg/day in divided doses.
Elderly
Dose adjustment may be necessary due to age-related decline in renal function. Start with a lower dose and titrate cautiously based on clinical response and tolerability.
Renal_impairment
Dose must be reduced in patients with impaired renal function. Dosage should be adjusted based on creatinine clearance. For example, for CrCl 30-59 mL/min, maximum daily dose is 400-800 mg/day.
How to Take
Take the capsule orally with a glass of water, with or without food. Do not chew, crush, or break the capsule. If taking antacids, take Gabatec at least 2 hours after the antacid.
Mechanism of Action
Gabapentin binds to the alpha-2-delta subunit of voltage-gated calcium channels in the brain. This action is thought to reduce the release of excitatory neurotransmitters, thereby decreasing neuronal excitability and controlling pain signals and seizures.

Onset of action for pain relief or seizure control can vary, often requiring gradual titration over several days to weeks to achieve full therapeutic effect.
Excretion
It is excreted unchanged by the kidneys. Renal elimination is the sole route of excretion.
Half life
Approximately 5-7 hours in individuals with normal renal function.
Absorption
Gabapentin is absorbed from the gastrointestinal tract via a saturable L-amino acid transport system. Bioavailability is dose-dependent, decreasing with increasing dose.
Metabolism
Gabapentin is not significantly metabolized in humans. It does not induce or inhibit hepatic enzymes.

- Hypersensitivity to gabapentin or any component of the formulation.
Drug Interactions
Antacids
Antacids containing aluminum and magnesium can reduce the bioavailability of gabapentin by about 20%. It is recommended to take gabapentin at least 2 hours after antacid administration.
Opioids (e.g., Morphine)
Concomitant use with opioids, especially morphine, can increase gabapentin levels and enhance CNS depressive effects like sedation, somnolence, and respiratory depression.
Alcohol and other CNS depressants
May enhance the sedative effects of gabapentin.

Store below 25°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, dizziness, blurred vision, slurred speech, lethargy, and mild diarrhea. Management is primarily supportive. Hemodialysis can be considered in severe cases, especially in patients with renal impairment.
Pregnancy & Lactation
Pregnancy Category C. Gabapentin has shown evidence of developmental toxicity in animals. It should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Gabapentin is excreted in human milk; caution should be exercised when administered to a nursing woman.
