Description
Overview of the medicine
Gabapentin is an anticonvulsant and neuropathic pain agent. It is used to treat partial seizures and neuropathic pain, such as postherpetic neuralgia and peripheral neuropathic pain.
Uses & Indications
Dosage
Adults
Initial dosage is typically 300 mg daily, titrated gradually to 900-1800 mg/day (or up to 3600 mg/day for some indications) in 3 divided doses. 75 mg strength is often used for initial titration or in specific patient populations.
Elderly
Dosage should be adjusted based on renal function, as gabapentin is renally excreted. Start with lower doses and titrate cautiously.
Renal_impairment
Dose reduction is required based on creatinine clearance. For CrCl 30-59 mL/min, maximum dose 700 mg/day; for CrCl 15-29 mL/min, maximum dose 400 mg/day; for CrCl <15 mL/min, maximum dose 200 mg/day.
How to Take
Take orally with or without food. Swallow the capsule whole with a glass of water. Do not crush, chew, or open the capsule.
Mechanism of Action
The exact mechanism of action of gabapentin is unknown. It is structurally related to the neurotransmitter GABA but does not bind to GABA-A or GABA-B receptors. It is believed to modulate the activity of voltage-gated calcium channels, leading to a reduction in excitatory neurotransmitter release.
Pharmacokinetics
Onset
Varies; for neuropathic pain, therapeutic effects may be seen within days to weeks; for seizures, it's typically within hours to days for initial effect.
Excretion
Excreted unchanged in the urine. Renal elimination is the sole route of elimination.
Half life
Approximately 5 to 7 hours in adults with normal renal function.
Absorption
Oral absorption is not dose-proportional, due to a saturable L-amino acid transport system. Bioavailability decreases with increasing dose.
Metabolism
Not significantly metabolized in humans; it does not induce hepatic microsomal enzymes.
Side Effects
Contraindications
- Known hypersensitivity to gabapentin or any component of the formulation.
Drug Interactions
Hydrocodone
Gabapentin may increase hydrocodone exposure.
Opioids (e.g., Morphine)
May increase gabapentin concentrations, leading to increased CNS depression. Monitor for sedation and respiratory depression.
Antacids (containing aluminum and magnesium)
May reduce gabapentin bioavailability. Take gabapentin at least 2 hours after antacids.
CNS depressants (e.g., benzodiazepines, alcohol)
Additive sedative effects and potential for respiratory depression. Use with caution.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, dizziness, ataxia, lethargy, blurred vision, slurred speech, and mild diarrhea. Management is primarily supportive; hemodialysis can be used for severe cases or in patients with renal impairment.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Gabapentin is excreted in breast milk; a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.

YesClinical Trials
Numerous clinical trials have demonstrated the efficacy and safety of gabapentin in the treatment of partial seizures and various neuropathic pain conditions. Studies confirm its role in reducing seizure frequency and pain intensity.
Lab Monitoring
- Monitor renal function, especially in elderly patients or those with pre-existing renal impairment.
- Monitor for signs of CNS depression or behavioral changes.
Doctor Notes
- Counsel patients about the potential for sedation, dizziness, and ataxia, especially during initiation and titration.
- Emphasize the importance of gradual withdrawal to prevent adverse events.
- Monitor for behavioral changes, including suicidal ideation, particularly in pediatric and adolescent patients.
Patient Guidelines
- Do not stop taking Gabapentin abruptly; gradual tapering is necessary to avoid withdrawal symptoms or increased seizure frequency.
- Report any unusual mood changes, suicidal thoughts, or new or worsening depression to your doctor immediately.
- Avoid alcohol and other CNS depressants while taking this medication as it may increase drowsiness and dizziness.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Gabapentin may cause dizziness, drowsiness, and impaired coordination. Do not drive or operate heavy machinery until you know how this medicine affects you.
Lifestyle Advice
- Maintain good hydration.
- Engage in light physical activity if possible.
