Description
Overview of the medicine
Lepsytam 500 mg Oral Solution contains Levetiracetam, an antiepileptic drug primarily used in the treatment of various types of seizures including partial onset seizures, myoclonic seizures, and primary generalized tonic-clonic seizures.
Uses & Indications
Dosage
Adults
Initial dose is typically 500 mg twice daily. This may be increased by 500 mg twice daily increments every 2 weeks up to a maximum of 1500 mg twice daily, depending on response and tolerability.
Elderly
Dosage adjustments may be necessary based on renal function. Initiate with a lower dose and titrate slowly.
Renal_impairment
Dosage reduction is required. Adjust dose based on creatinine clearance (e.g., lower daily dose or extended dosing interval).
How to Take
Administer orally, usually twice daily, with or without food. Use a calibrated measuring device to ensure accurate dosing. Do not dilute the oral solution.
Mechanism of Action
The exact mechanism of action of Levetiracetam is not fully understood, but it is believed to involve selective binding to the synaptic vesicle protein 2A (SV2A) in the brain, which may modulate neurotransmitter release and prevent excessive neuronal excitability.
Pharmacokinetics
Onset
Rapid, typically within hours of the first dose.
Excretion
Primarily excreted via the kidneys, with approximately 66% of the dose excreted unchanged in urine.
Half life
Approximately 7-9 hours in adults, shorter in children (5-6 hours).
Absorption
Rapid and almost complete absorption after oral administration. Peak plasma concentrations are usually reached within 1 hour.
Metabolism
Minimal metabolism, not extensively metabolized by hepatic cytochrome P450 system. Primarily hydrolyzed into an inactive metabolite.
Side Effects
Contraindications
- •Known hypersensitivity to Levetiracetam or any other components of the formulation, or other pyrrolidone derivatives.
Drug Interactions
Phenytoin
Levetiracetam does not significantly alter the pharmacokinetics of common antiepileptic drugs like phenytoin. However, some evidence suggests a minor decrease in Levetiracetam clearance with enzyme-inducing AEDs, usually not clinically significant.
Central Nervous System (CNS) Depressants
Concurrent use with alcohol or other CNS depressants may lead to additive sedative effects and increased dizziness or drowsiness.
Storage
Store below 25°C. Do not freeze. Keep the bottle tightly closed in the original package. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, agitation, aggression, depressed level of consciousness, respiratory depression, and coma. Management is primarily supportive, including gastric lavage or activated charcoal if recent ingestion, and symptomatic treatment. Hemodialysis can remove Levetiracetam effectively.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs risk. Levetiracetam is excreted into breast milk; caution advised. Consult a doctor before use during pregnancy or breastfeeding.
