Description
Overview of the medicine
Lepsytam 500 mg tablet contains Levetiracetam, an antiepileptic drug used to control and prevent various types of seizures. It stabilizes the activity of the central nervous system.
Uses & Indications
Dosage
Adults
Initial dose 500 mg twice daily, may be increased by 500 mg twice daily every 2 weeks to a maximum of 1500 mg twice daily (3000 mg/day).
Elderly
Similar to adults, but dose adjustment may be necessary based on renal function.
Renal_impairment
Dose reduction is required; adjusted based on creatinine clearance (e.g., 250-750 mg once daily for mild to moderate impairment).
How to Take
Take orally, with or without food. Swallow the tablet whole with a glass of water. Do not crush or chew.
Mechanism of Action
Levetiracetam binds to synaptic vesicle protein 2A (SV2A) which is thought to modulate neurotransmitter release and stabilize neuronal hyperexcitability, thus preventing seizures.

Onset
Relatively rapid.
Excretion
Primarily renal excretion (66% unchanged, 24% inactive metabolite).
Half life
6-8 hours in adults; shorter in children, prolonged in renal impairment.
Absorption
Rapid and almost complete absorption. Oral bioavailability is approximately 100%.
Metabolism
Not extensively metabolized by hepatic CYP enzymes; undergoes enzymatic hydrolysis of the acetamide group (major inactive metabolite).
Side Effects
Contraindications
- •Hypersensitivity to levetiracetam or other pyrrolidone derivatives.
Drug Interactions
Methotrexate
May reduce clearance of methotrexate, leading to increased methotrexate concentrations. Monitor for toxicity.
Oral Contraceptives
Levetiracetam does not significantly affect the pharmacokinetics of oral contraceptives.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, agitation, aggression, depressed consciousness, respiratory depression, and coma. Management involves symptomatic and supportive treatment; hemodialysis can remove up to 50% of levetiracetam.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs risk. Levetiracetam is excreted in breast milk; caution is advised for nursing mothers.
