Description
Overview of the medicine
Levofloxacin is a broad-spectrum fluoroquinolone antibiotic used to treat a variety of bacterial infections.
Uses & Indications
Dosage
Adults
Typically 500 mg once daily for 7-14 days depending on the infection. For severe infections, 750 mg once daily may be used.
Elderly
No specific dose adjustment for elderly patients based solely on age, but renal function should be assessed and dose adjusted if necessary.
Renal_impairment
Dosage adjustment is required for patients with creatinine clearance < 50 mL/min. For example, 250 mg every 48 hours for severe impairment (CrCl 10-19 mL/min).
How to Take
Libott tablets can be taken with or without food. Swallow the tablet whole with a sufficient amount of water. Do not crush or chew.
Mechanism of Action
Levofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes essential for bacterial DNA replication, transcription, repair, and recombination, leading to bacterial cell death.
Pharmacokinetics
Onset
Clinical effects typically observed within 1-2 hours, with full therapeutic effect over several days.
Excretion
Primarily excreted unchanged in the urine, mainly by glomerular filtration and tubular secretion.
Half life
Approximately 6-8 hours, allowing for once-daily dosing.
Absorption
Rapid and complete absorption after oral administration, with peak plasma concentrations reached within 1-2 hours.
Metabolism
Minimally metabolized in the liver; less than 5% of an oral dose is metabolized.
Side Effects
Contraindications
- •Hypersensitivity to levofloxacin or other fluoroquinolones
- •History of tendinitis or tendon rupture associated with fluoroquinolone use
- •Epilepsy
- •Children and adolescents (under 18 years) due to risk of cartilage damage
- •Pregnant or breastfeeding women
Drug Interactions
NSAIDs
May increase the risk of CNS stimulation and convulsive seizures.
Warfarin
May enhance the anticoagulant effect of warfarin, requiring close monitoring of INR.
Corticosteroids
Increased risk of severe tendon disorders (tendinitis and tendon rupture).
Antiarrhythmics (e.g., quinidine, procainamide), tricyclic antidepressants
May increase the risk of QT interval prolongation.
Antacids (containing magnesium, aluminum), sucralfate, iron/zinc supplements
Can significantly reduce absorption; administer Libott at least 2 hours before or 2 hours after these agents.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach and sight of children.
Overdose
Symptoms of overdose include central nervous system effects (e.g., confusion, dizziness, seizures), prolongation of the QT interval, and gastrointestinal reactions such as nausea and erosions of mucous membranes. Management is supportive and symptomatic, including gastric lavage and adequate hydration.
Pregnancy & Lactation
Pregnancy Category C. Not recommended during pregnancy as animal studies show adverse effects. Not recommended during breastfeeding due to the potential for serious adverse reactions, including cartilage damage, in infants.
