Description
Overview of the medicine
Lutisone 50 mcg Nasal Spray is a corticosteroid medication used to relieve symptoms of seasonal and perennial allergic rhinitis, such as sneezing, runny nose, itchy nose, and nasal congestion. It may also be used to treat nasal polyps.
Uses & Indications
Dosage
Adults
For allergic rhinitis: 2 sprays per nostril once daily (200 mcg total daily dose). Some patients may require 2 sprays per nostril twice daily (400 mcg total daily dose) initially, then reduce to once daily. For nasal polyps: 2 sprays per nostril twice daily (400 mcg total daily dose).
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment.
How to Take
Shake the bottle gently before each use. Prime the pump before the first use or if it hasn't been used for several days. Tilt your head forward slightly, insert the nozzle into one nostril, aiming slightly away from the septum, and press the pump while sniffing gently. Repeat for the other nostril. Avoid spraying into the eyes.
Mechanism of Action
Lutisone (Fluticasone Propionate) is a synthetic trifluorinated corticosteroid that possesses potent anti-inflammatory activity. It works by reducing inflammation in the nasal passages, thereby alleviating symptoms like sneezing, itching, runny nose, and congestion. It inhibits multiple cell types and mediators involved in allergic and non-allergic mediated inflammation.
Pharmacokinetics
Onset
Symptomatic relief may begin within 12 hours, but full benefit typically takes several days to a week of regular use.
Excretion
Excreted primarily in the feces (as metabolites), with less than 5% excreted in the urine.
Half life
Approximately 7-8 hours (for the small systemically absorbed portion).
Absorption
Low systemic absorption (typically <2%) following intranasal administration, primarily due to first-pass metabolism in the liver and presystemic metabolism in the gut.
Metabolism
Extensively metabolized in the liver via cytochrome P450 3A4 (CYP3A4) enzyme to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Lutisone or any component of the formulation.
- Untreated localized infection involving the nasal mucosa.
Drug Interactions
Ritonavir
Concomitant use with strong CYP3A4 inhibitors like ritonavir can significantly increase systemic exposure to Lutisone, leading to potential corticosteroid systemic effects. Co-administration should be avoided unless the potential benefit outweighs the risk of systemic corticosteroid side effects.
Ketoconazole
Other strong CYP3A4 inhibitors (e.g., ketoconazole) may also increase systemic exposure, but to a lesser extent than ritonavir. Use with caution.
Storage
Store upright at room temperature (15°C to 30°C). Protect from freezing and light.
Overdose
Acute overdose is unlikely due to the low systemic absorption. Chronic overdose or excessive doses may lead to signs and symptoms of hypercorticism or adrenal suppression. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known if Lutisone is excreted in human milk; use with caution in nursing mothers.
