Description
Overview of the medicine
Pregabalin is an anticonvulsant and analgesic medication used to treat neuropathic pain, fibromyalgia, generalized anxiety disorder, and as an adjunctive therapy for partial seizures.
Uses & Indications
Dosage
Adults
Starting dose typically 25-75 mg/day, gradually increased over a week or more based on response and tolerability, up to a maximum of 600 mg/day in 2-3 divided doses. Specific dose depends on indication.
Elderly
Dose adjustment may be required in elderly patients due to age-related decline in renal function. Initial doses should be lower.
Renal_impairment
Dose adjustment required based on creatinine clearance. For CrCl 30-60 mL/min, initial dose 25 mg once daily or 25 mg twice daily; maximum 300 mg/day.
How to Take
Lyric capsules should be taken orally, with or without food. Swallow the capsule whole with water.
Mechanism of Action
Binds to the alpha-2-delta subunit of voltage-gated calcium channels in the central nervous system, reducing the release of several neurotransmitters like glutamate, norepinephrine, and substance P. This results in reduced neuronal excitability and pain transmission.
Pharmacokinetics
Onset
Onset of pain relief typically within 1-2 hours; for anxiety, effects may take longer to manifest.
Excretion
Primarily excreted unchanged in the urine (approximately 98% of the administered dose).
Half life
The elimination half-life is approximately 6.3 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract, with bioavailability of ≥90%. Peak plasma concentrations (Tmax) are reached within approximately 1 hour.
Metabolism
Undergoes negligible hepatic metabolism in humans.
Side Effects
Contraindications
- Hypersensitivity to pregabalin or any of the excipients in the formulation.
Drug Interactions
Ethanol (Alcohol)
Potentiates the impairing effects of ethanol on motor and cognitive function.
Benzodiazepines (e.g., Lorazepam)
Additive CNS depressant effects when taken with pregabalin.
Opioids (e.g., Oxycodone, Morphine)
Increased risk of CNS depression and respiratory depression when co-administered with pregabalin. Close monitoring is recommended.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, confusion, agitation, and restlessness. In rare cases, coma and seizures have been reported. Management is supportive, and hemodialysis may be considered for severe cases.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Pregabalin is excreted in human milk; a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.

Lab Monitoring
- Renal function monitoring (creatinine clearance) is important, especially in elderly patients and those with known renal impairment.
Doctor Notes
- Counsel patients on the potential for abuse and dependence, and the importance of gradual dose tapering upon discontinuation.
- Monitor for signs of suicidal ideation and unusual behavioral changes, especially at the initiation of therapy or during dose adjustments.
- Dose adjustments are crucial for patients with renal impairment to avoid accumulation and increased side effects.
- Advise patients about potential CNS depressant effects and interactions with alcohol or other CNS depressants.
Patient Guidelines
- Do not stop Lyric suddenly. The dose should be tapered gradually over at least one week to reduce the risk of withdrawal symptoms.
- This medicine may cause dizziness or drowsiness. Avoid driving or operating heavy machinery until you know how it affects you.
- Avoid alcohol while taking this medication, as it can increase side effects like dizziness and drowsiness.
- Report any unusual mood changes, suicidal thoughts, or signs of allergic reactions (e.g., swelling of face, lips, tongue) to your doctor immediately.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Lyric may cause dizziness, somnolence, blurred vision, or impaired coordination. Patients should be advised not to drive or operate complex machinery or engage in other potentially dangerous activities until they have gained sufficient experience with Lyric to gauge whether it adversely affects their ability to perform such tasks.
