Overview of the medicine
Macrozith (Azithromycin) is a macrolide antibiotic used to treat various bacterial infections, including respiratory tract infections, skin and soft tissue infections, and sexually transmitted diseases caused by susceptible organisms.
Uses & Indications
Dosage
Adults
For most infections: 500 mg once daily for 3 days or 500 mg on Day 1, then 250 mg once daily for 4 days. For Chlamydia trachomatis: 1 g as a single dose.
Elderly
No dosage adjustment generally required, but monitor for increased susceptibility to adverse effects, especially QT prolongation.
Renal_impairment
No dosage adjustment necessary for mild to moderate renal impairment. Use with caution in severe impairment as data is limited.
How to Take
Macrozith tablets can be taken with or without food. Macrozith oral suspension can also be taken with or without food; shake the suspension well before each use.
Mechanism of Action
Azithromycin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit, thereby blocking translocation of peptides. It is primarily bacteriostatic but can be bactericidal at high concentrations against susceptible organisms.

Onset
Peak plasma concentrations reached within 2-3 hours.
Excretion
Primarily excreted unchanged in the bile; a small amount is excreted in the urine.
Half life
Terminal elimination half-life is prolonged, ranging from 2 to 4 days.
Absorption
Rapidly absorbed from the gastrointestinal tract; bioavailability is approximately 37%. Food decreases absorption of tablets.
Metabolism
Primarily metabolized in the liver to inactive metabolites, but systemic metabolism is minimal.
Side Effects
Contraindications
- •Hypersensitivity to azithromycin, erythromycin, any macrolide or ketolide antibiotic, or any component of the formulation.
- •History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use.
Drug Interactions
Warfarin
Increased anticoagulant effect; monitor INR closely.
Nelfinavir
Increased azithromycin concentrations; monitor for side effects.
Antacids (containing aluminum or magnesium)
Decreases peak plasma levels; take azithromycin at least 1 hour before or 2 hours after antacids.
QT prolonging drugs (e.g., Class IA and III antiarrhythmics, antipsychotics, antidepressants)
Increased risk of QT prolongation and Torsades de Pointes.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children. Reconstituted suspension should be stored as per manufacturer's instructions, typically at room temperature or refrigerated, and discarded after a specific period.
Overdose
Symptoms of overdose may include severe nausea, vomiting, diarrhea, and reversible hearing loss. Management involves gastric lavage, supportive care, and monitoring of vital signs and ECG.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus. Use during pregnancy only if clearly needed. Azithromycin is excreted into breast milk; use with caution in nursing mothers.
