Overview of the medicine
Maxpro 20 mg Capsule contains Esomeprazole, a proton pump inhibitor that reduces the amount of acid produced in the stomach. It is used to treat conditions such as gastroesophageal reflux disease (GERD), peptic ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
GERD: 20-40 mg once daily for 4-8 weeks. Erosive Esophagitis: 20-40 mg once daily for 4-8 weeks. Maintenance: 20 mg once daily. H. pylori: 20 mg twice daily for 7-10 days (with antibiotics). Zollinger-Ellison: 40 mg twice daily, adjusted as needed.
Elderly
No specific dosage adjustment is generally required, but caution is advised in patients with severe hepatic impairment.
Renal_impairment
No dosage adjustment is needed for patients with renal impairment.
How to Take
Take the capsule orally, at least one hour before a meal. Swallow whole with water; do not chew or crush. For patients who have difficulty swallowing, the capsule can be opened and the pellets mixed with a small amount of water or soft food.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that specifically inhibits the H+/K+-ATPase enzyme system (the proton pump) in the gastric parietal cells. This inhibition prevents the final step of acid secretion, thereby reducing gastric acidity.

Absorption

Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, mainly by CYP2C19 and CYP3A4.

Contraindications
- Hypersensitivity to Esomeprazole, substituted benzimidazoles, or any component of the formulation
- Concomitant use with nelfinavir

Warfarin
Increased INR and prothrombin time; monitor closely.
Clopidogrel
May reduce the antiplatelet effect of clopidogrel; consider alternative antiplatelet therapy.
Methotrexate
Increased methotrexate levels; monitor for toxicity.
Ketoconazole, Itraconazole
Reduced absorption of these drugs due to increased gastric pH.

Limited experience with overdose. Symptoms may include drowsiness, tachycardia, nausea, diarrhea, headache, and sweating. Treatment is symptomatic and supportive. Esomeprazole is not readily dialyzable.
