Description
Overview of the medicine
Maxpro 40 mg Capsule contains Esomeprazole, a proton pump inhibitor, used to reduce stomach acid production. It treats conditions like acid reflux, ulcers, and heartburn.
Uses & Indications
Dosage
Adults
GERD: 20-40 mg once daily for 4-8 weeks. Erosive Esophagitis: 20-40 mg once daily for 4-8 weeks. H. pylori Eradication: 40 mg once daily for 10 days (with antibiotics). Zollinger-Ellison Syndrome: 40 mg twice daily initially, adjusted as needed.
Elderly
No dose adjustment is generally required for elderly patients.
Renal_impairment
No dose adjustment is generally required for patients with renal impairment.
How to Take
Take the capsule whole, at least one hour before a meal, usually in the morning. Do not chew or crush. For patients who have difficulty swallowing, the capsule can be opened and the pellets mixed with a small amount of water or soft food (e.g., apple sauce) and swallowed immediately. Do not chew or crush the pellets.
Mechanism of Action
Esomeprazole selectively inhibits the gastric proton pump (H+/K+-ATPase) in parietal cells of the stomach, thereby blocking the final step of acid secretion. This leads to a dose-dependent inhibition of both basal and stimulated gastric acid secretion.
Pharmacokinetics
Onset
Within 1 hour
Excretion
Mainly excreted as metabolites in urine (approx. 80%) and feces (approx. 20%).
Half life
Approximately 1-1.5 hours
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to esomeprazole or any components of the formulation or to other substituted benzimidazoles.
- Concomitant use with nelfinavir.
Drug Interactions
Digoxin
Increased absorption of digoxin due to increased gastric pH.
Warfarin
Increased INR and prothrombin time, potentially leading to bleeding.
Clopidogrel
Potential reduction in antiplatelet effect of clopidogrel (clinical significance debated).
Methotrexate
Possible increased systemic exposure to methotrexate (high-dose).
Nelfinavir, Atazanavir
Decreased plasma levels of these antiretrovirals due to increased gastric pH.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are generally mild and include confusion, drowsiness, blurred vision, tachycardia, nausea, sweating, flushing, headache, dry mouth, and other gastrointestinal disturbances. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown whether esomeprazole is excreted in human milk; caution should be exercised when administered to a nursing woman.
