Description
Overview of the medicine
Esomeprazole is a proton pump inhibitor that reduces the amount of acid produced in the stomach. It is used to treat conditions like GERD, ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
GERD/Erosive Esophagitis: 20-40 mg once daily for 4-8 weeks. Zollinger-Ellison Syndrome: Initial 40 mg twice daily. H. pylori eradication: 20 mg twice daily for 7-14 days with appropriate antibiotics.
Elderly
No specific dose adjustment is generally required, but caution is advised in elderly patients due to potential comorbidities.
Renal_impairment
No dose adjustment is necessary for patients with renal impairment.
How to Take
Swallow the tablet whole with water, preferably at least one hour before a meal. Do not chew or crush.
Mechanism of Action
Esomeprazole selectively inhibits the H+/K+-ATPase enzyme system (the proton pump) at the secretory surface of the gastric parietal cell, thereby blocking the final step of acid secretion.
Pharmacokinetics
Onset
Within 1 hour
Excretion
Primarily excreted in urine (80%) and feces (20%).
Half life
Approximately 1.5 hours
Absorption
Rapidly absorbed after oral administration, with bioavailability of approximately 64% (single dose) to 89% (repeated doses).
Metabolism
Extensively metabolized in the liver by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to esomeprazole or any components of the formulation
- Concomitant use with nelfinavir
Drug Interactions
Diazepam
Decreased clearance of diazepam, leading to increased plasma concentrations.
Warfarin
Increased INR and prothrombin time, potentially leading to abnormal bleeding. Monitor closely.
Clopidogrel
May reduce the antiplatelet effect of clopidogrel. Avoid concomitant use.
Drugs dependent on gastric pH (e.g., ketoconazole, iron salts)
Altered absorption due to increased gastric pH.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptoms are generally mild and reversible. Treatment should be symptomatic and supportive. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category B. Should be used during pregnancy only if clearly needed. Esomeprazole is excreted in human breast milk; use with caution in breastfeeding mothers.
