Maxpro 40 mg Injection contains Esomeprazole, a proton pump inhibitor, used to reduce the amount of acid in your stomach. It is typically used for conditions where too much stomach acid is produced, or when oral medication is not feasible.
Uses & Indications
Dosage
Adults
40 mg once daily by intravenous infusion over a period of 10 to 30 minutes. In Zollinger-Ellison Syndrome, the initial dose is 80 mg once daily, adjusted as needed.
Elderly
No dose adjustment is generally required, but caution in patients with severe hepatic impairment.
Renal_impairment
No dose adjustment is necessary for patients with impaired renal function.
How to Take
Administered as an intravenous infusion over 10-30 minutes. The reconstituted solution should be clear and colorless to pale yellow.
Mechanism of Action
Esomeprazole selectively inhibits the H+/K+-ATPase (proton pump) in the secretory canaliculi of the gastric parietal cells. This irreversible blockade prevents the final step of acid production, leading to a sustained reduction in gastric acid secretion.

Onset
Acid inhibition occurs within one hour of administration.
Excretion
Approximately 80% of metabolites are excreted in the urine, and the remaining 20% in the feces.
Half life
Approximately 1-1.5 hours in healthy volunteers.
Absorption
Rapidly absorbed after intravenous administration. Bioavailability is 100%.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, mainly by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir (an antiretroviral).
Drug Interactions
Diazepam
Esomeprazole may inhibit the metabolism of diazepam, increasing its plasma concentration.
Warfarin
Increased INR and bleeding risk reported with concomitant use.
Clopidogrel
Esomeprazole may reduce the antiplatelet effect of clopidogrel.
Methotrexate
Possible increase in methotrexate levels, especially with high-dose therapy.
