Description
Overview of the medicine
Monocast 10 mg Tablet contains Montelukast, a leukotriene receptor antagonist used for the prophylaxis and chronic treatment of asthma and for the relief of symptoms of seasonal and perennial allergic rhinitis.
Uses & Indications
Dosage
Adults
For asthma and allergic rhinitis: 10 mg once daily, in the evening.
Elderly
No dosage adjustment generally required.
Renal_impairment
No dosage adjustment generally required.
How to Take
Take orally once daily in the evening, with or without food. For asthma, take in the evening. For allergic rhinitis, timing may be adjusted to suit patient needs. For exercise-induced bronchoconstriction, take at least 2 hours before exercise, not to exceed once daily.
Mechanism of Action
Montelukast selectively blocks the cysteinyl leukotriene 1 (CysLT1) receptor, which prevents the bronchoconstrictor, pro-inflammatory, and other effects of leukotrienes. Leukotrienes are potent inflammatory mediators released from various cells, including mast cells and eosinophils, involved in asthma and allergic rhinitis.
Pharmacokinetics
Onset
Clinical improvement usually observed within 1 day (for asthma and allergic rhinitis).
Excretion
Excreted primarily in the feces via bile (approximately 86%), with less than 0.2% excreted in urine.
Half life
Approximately 2.7 to 5.5 hours.
Absorption
Rapidly absorbed after oral administration; peak plasma concentration (Tmax) typically reached in 2-4 hours for tablets.
Metabolism
Extensively metabolized in the liver by cytochrome P450 enzymes (CYP3A4, CYP2C9, CYP2A6).
Side Effects
Contraindications
- •Hypersensitivity to Montelukast or any component of the formulation.
Drug Interactions
Rifampicin
May decrease Montelukast plasma concentrations, leading to reduced efficacy.
Gemfibrozil
May increase Montelukast plasma concentrations (minor interaction).
Phenobarbital
May decrease Montelukast plasma concentrations.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose often include abdominal pain, somnolence, thirst, headache, vomiting, and psychomotor hyperactivity. Treatment is symptomatic and supportive. No specific antidote is known.
Pregnancy & Lactation
Pregnancy category B. Use during pregnancy only if clearly needed. Montelukast is excreted in breast milk; caution should be exercised when administered to a nursing woman.
