Monocast 4 mg Chewable Tablet contains Montelukast, a leukotriene receptor antagonist used for the maintenance treatment of asthma and to relieve symptoms of seasonal and perennial allergic rhinitis. The 4 mg strength is typically prescribed for children aged 2-5 years.
Uses & Indications
Dosage
Adults
Not typically used for adults; higher strengths (e.g., 10 mg tablet) are usually prescribed for adults.
Elderly
Not typically used for elderly; higher strengths (e.g., 10 mg tablet) are usually prescribed for elderly patients.
Pediatric
Children 2 to 5 years of age: 4 mg once daily in the evening.

How to Take
For oral use. Chew the tablet completely before swallowing. Take once daily in the evening, with or without food.
Mechanism of Action
Montelukast selectively antagonizes the CysLT1 receptor, preventing the actions of leukotrienes (LTC4, LTD4, LTE4). Leukotrienes are potent inflammatory mediators released from various cells, including mast cells and eosinophils, and contribute to airway obstruction, increased mucus secretion, and other asthma symptoms, as well as allergic rhinitis symptoms.
Pharmacokinetics
Onset
Within one day for asthma and allergic rhinitis.
Excretion
Primarily excreted in feces via bile (approximately 86%), with less than 0.2% excreted in urine.
Half life
Approximately 2.7 to 5.5 hours.
Absorption
Rapidly absorbed after oral administration. Bioavailability is approximately 64% for the chewable tablet. Peak plasma concentrations are reached in 2-4 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP3A4, CYP2C9, and CYP2C8.

Contraindications

Drug Interactions
Rifampin
May decrease Montelukast plasma concentrations.
Phenobarbital
May decrease Montelukast plasma concentrations.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
No specific antidote. Management should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B (US FDA). Use only if clearly needed. Unknown if excreted in human milk; use with caution.
