Description
Overview of the medicine
Monocast 4 mg Oral Granules contain Montelukast, a leukotriene receptor antagonist. It is primarily used for the management of asthma and allergic rhinitis in pediatric patients aged 6 months to 5 years. It helps to prevent symptoms like wheezing, shortness of breath, and chest tightness caused by asthma, and also relieves symptoms of seasonal and perennial allergic rhinitis such as sneezing, runny nose, and nasal congestion.

Dosage
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment.
Hepatic_impairment
No dosage adjustment is necessary for patients with mild to moderate hepatic impairment. For severe hepatic impairment, data is not available.
Children_6_months_to_5_years
One 4 mg sachet once daily in the evening.
How to Take
Monocast 4 mg Oral Granules can be administered directly into the mouth, or mixed with a spoonful of soft food (e.g., applesauce, mashed carrots, rice, or ice cream) at room temperature. The granules should not be dissolved in liquid. Do not open the sachet until ready to use. Administer within 15 minutes of opening the sachet. Do not store for future use. It is usually taken once daily in the evening for asthma and perennial allergic rhinitis, or at a time convenient for seasonal allergic rhinitis.
Mechanism of Action
Montelukast selectively blocks the cysteinyl leukotriene type 1 (CysLT1) receptor. Leukotrienes are inflammatory mediators released from cells such as mast cells and eosinophils, contributing to bronchoconstriction, mucus secretion, vascular permeability, and eosinophil recruitment in the airways. By blocking these receptors, Montelukast inhibits the actions of leukotrienes, leading to reduced airway inflammation, improved lung function, and alleviation of allergic rhinitis symptoms.
Pharmacokinetics
Onset
Clinical improvement in asthma symptoms seen within 1 day.
Excretion
Primarily excreted in feces via bile (>86%), with minimal renal excretion (<0.2%).
Half life
Plasma half-life ranges from 2.7 to 5.5 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentration (Tmax) is reached in about 2-4 hours for granules.
Metabolism
Extensively metabolized in the liver by CYP3A4, 2C8, and 2C9 enzymes. Metabolites are pharmacologically inactive.
Side Effects
Contraindications
- •Hypersensitivity to Montelukast or any component of the formulation.
- •Monocast is not indicated for use in the reversal of bronchospasm in acute asthma attacks.
Drug Interactions
Rifampicin
May decrease the plasma concentration of Montelukast, requiring careful monitoring.
Phenobarbital
May decrease the area under the plasma concentration-time curve (AUC) of Montelukast by approximately 40%.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children. Do not open the sachet until ready to use.
Overdose
In most overdose reports, no adverse events were reported. The most common adverse events were thirst, somnolence, mydriasis, hyperkinesia, and abdominal pain. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus. There are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Montelukast is excreted into breast milk. Caution should be exercised when administered to a nursing mother.
