Description
Overview of the medicine
Monocast 5 mg Chewable Tablet contains Montelukast, a leukotriene receptor antagonist used for the treatment of asthma and allergic rhinitis. It helps to prevent wheezing and shortness of breath caused by asthma, and relieve symptoms of allergic rhinitis like sneezing, runny nose, and itchy nose.
Uses & Indications
Dosage
Adults
For adults, the usual dose is 10 mg once daily. 5 mg chewable tablets are primarily for children 6-14 years.
Elderly
No dosage adjustment needed for elderly patients taking 10 mg. 5 mg chewable tablets are primarily for children 6-14 years.
Renal_impairment
No dosage adjustment needed for any age group.
How to Take
The chewable tablet should be chewed thoroughly before swallowing. It can be taken with or without food. For asthma and allergic rhinitis, take once daily in the evening.
Mechanism of Action
Montelukast selectively antagonizes the CysLT1 receptor, inhibiting the physiological actions of leukotriene D4 (LTD4) at this receptor. Leukotrienes are potent inflammatory mediators involved in the pathophysiology of asthma and allergic rhinitis.

Onset
Asthma symptoms improve within 1 day of starting treatment.
Excretion
Primarily excreted in feces via bile (approximately 86%), with less than 0.2% excreted in urine.
Half life
Approximately 2.7 to 5.5 hours.
Absorption
Rapidly absorbed after oral administration; bioavailability of a 5 mg chewable tablet is 73% (compared to 66% for a 10 mg film-coated tablet).
Metabolism
Extensively metabolized by cytochrome P450 enzymes (CYP3A4, CYP2C9, and CYP2C8) in the liver.

Contraindications

Drug Interactions
Rifampicin
May decrease Montelukast levels due to enzyme induction.
Phenobarbital
May decrease Montelukast levels, clinical significance unknown.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
No specific antidote. Symptoms of overdose include abdominal pain, somnolence, thirst, headache, vomiting, and psychomotor hyperactivity. Supportive care is recommended.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Montelukast is excreted in breast milk; use with caution in nursing mothers.
