Moxifloxacin is a broad-spectrum fluoroquinolone antibiotic used to treat various bacterial infections, including respiratory, skin, and intra-abdominal infections. The 400 mg injection is administered intravenously.
Uses & Indications
Dosage
Adults
400 mg administered once daily by intravenous infusion over 60 minutes.
Elderly
No dosage adjustment usually required based on age alone.
Renal_impairment
No dosage adjustment required for mild to severe renal impairment or in patients on hemodialysis or CAPD.
How to Take
Administer intravenously over a period of 60 minutes. Do not administer as a rapid or bolus injection.
Mechanism of Action
Moxifloxacin inhibits bacterial DNA gyrase and topoisomerase IV, essential enzymes involved in bacterial DNA replication, transcription, repair, and recombination, leading to bacterial cell death.

Onset
Rapid (within minutes for IV administration).
Excretion
Excreted via both renal (approx. 20%) and fecal/biliary (approx. 25%) routes as unchanged drug and metabolites.
Half life
Approximately 12-15 hours.
Absorption
Rapidly and almost completely absorbed when administered orally; IV administration bypasses absorption.
Metabolism
Partially metabolized in the liver via Phase II glucuronide and sulfate conjugation.
Side Effects
Contraindications
- Hypersensitivity to moxifloxacin or other quinolones
- History of tendinitis or tendon rupture associated with fluoroquinolone use
- QTc prolongation
- Uncorrected hypokalemia
Drug Interactions
Warfarin
May potentiate anticoagulant effect.
QTc-prolonging drugs
Increased risk of QTc prolongation (e.g., Class IA and III antiarrhythmics, tricyclic antidepressants, macrolides, antipsychotics).
Antacids, Iron, Zinc, Sucralfate, Didanosine
May reduce absorption if given orally (less relevant for IV but still a general drug interaction).

In case of overdose, symptomatic and supportive treatment should be initiated. ECG monitoring is recommended due to the risk of QTc prolongation. Hemodialysis or peritoneal dialysis is not effective in removing moxifloxacin.
