Description
Overview of the medicine
Moxifloxacin is a fluoroquinolone antibiotic used to treat various bacterial infections, including respiratory tract infections, skin and soft tissue infections, and intra-abdominal infections.
Uses & Indications
Dosage
Adults
400 mg orally once daily for 5-21 days, depending on the infection.
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment (including those on hemodialysis or CAPD).
How to Take
Take orally, with or without food. Tablets should be swallowed whole with sufficient liquid and not chewed.
Mechanism of Action
Moxifloxacin inhibits bacterial DNA gyrase and topoisomerase IV, essential enzymes involved in DNA replication, transcription, repair, and recombination. This leads to disruption of bacterial DNA and cell death.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved within 0.5 to 4 hours.
Excretion
Excreted both renally (approximately 20% as unchanged drug) and through the biliary/fecal route (approximately 25% as unchanged drug).
Half life
Approximately 12 hours.
Absorption
Rapid and almost complete absorption from the gastrointestinal tract. Bioavailability is approximately 90%.
Metabolism
Undergoes hepatic metabolism (Phase II, non-CYP) to form inactive sulfate and glucuronide conjugates.
Side Effects
Contraindications
- Hypersensitivity to moxifloxacin or other quinolones.
- History of tendinitis or tendon rupture associated with quinolone use.
- QTc prolongation or family history of QTc prolongation.
- Uncorrected hypokalemia or hypomagnesemia.
- Severe hepatic impairment (Child-Pugh Class C).
Drug Interactions
Warfarin
Increased anticoagulant effect; monitor INR closely.
Antacids, iron, zinc, sucralfate
May reduce absorption of moxifloxacin; administer moxifloxacin several hours before or after these agents.
Class IA and III Antiarrhythmics (e.g., quinidine, amiodarone)
Increased risk of QTc prolongation; concomitant use is contraindicated.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.

In case of an acute overdose, supportive measures should be initiated. ECG monitoring is recommended due to the potential for QTc prolongation. Activated charcoal may be used for oral overdose if administered shortly after ingestion.
Pregnancy & Lactation
Moxifloxacin should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in human milk, and a decision should be made whether to discontinue nursing or discontinue the drug, considering the importance of the drug to the mother.
