Description
Overview of the medicine
Nerfid 25 mg capsule is an antidepressant and anxiolytic medication used to treat major depressive disorder and generalized anxiety disorder. It belongs to the class of serotonin-norepinephrine reuptake inhibitors (SNRIs).
Uses & Indications
Dosage
Adults
Initial dose 25 mg once daily, may be increased to 50 mg daily after 1-2 weeks based on response. Max 100 mg/day.
Elderly
Start with 25 mg once daily, monitor closely for adverse effects. Max 50 mg/day.
Renal_impairment
Mild to moderate: No dose adjustment needed. Severe: Reduce dose by 50%.
How to Take
Take orally with or without food, preferably at the same time each day.
Mechanism of Action
Nerfid selectively inhibits the reuptake of both serotonin and norepinephrine in the central nervous system, thereby increasing the concentrations of these neurotransmitters in the synaptic cleft. This leads to an improvement in mood and a reduction in anxiety symptoms.

Initial antidepressant effects may be seen within 1-2 weeks; full effects within 4-6 weeks.
Excretion
Mainly renal, with about 70% excreted as metabolites and 5% as unchanged drug within 72 hours.
Half life
Approximately 10-15 hours.
Absorption
Well absorbed orally, with peak plasma concentrations reached within 2-4 hours.
Metabolism
Primarily hepatic via CYP2D6 and CYP3A4, with active and inactive metabolites.
