Description
Overview of the medicine
Nerfid 50 mg Capsule is a medication used to treat neuropathic pain conditions such as diabetic neuropathy or postherpetic neuralgia. It works by reducing abnormal nerve activity, thereby alleviating pain sensations.
Uses & Indications
Dosage
Adults
Initially 50 mg once daily, titrated up to 300 mg/day in divided doses. Maximum recommended dose is 600 mg/day.
Elderly
Start with a lower dose (e.g., 25 mg daily) and titrate cautiously due to potential for reduced renal function.
Renal_impairment
Dose reduction required based on creatinine clearance. Consult prescribing information for specific guidelines.
How to Take
Take orally, with or without food. Swallow the capsule whole with water. Do not crush or chew.
Mechanism of Action
Nerfid works by binding to the alpha-2-delta subunit of voltage-gated calcium channels in the central nervous system, thereby reducing the release of excitatory neurotransmitters from nerve cells that contribute to pain.
Pharmacokinetics
Onset
Initial effects may be observed within 1-2 hours, but full therapeutic effect may take several days.
Excretion
Mainly excreted unchanged in the urine via renal excretion.
Half life
The elimination half-life is typically 5-7 hours.
Absorption
Rapidly absorbed after oral administration, with an average bioavailability of approximately 60%. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Undergoes minimal hepatic metabolism. Primarily excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to Nerfid or any excipients
- Acute pancreatitis
Drug Interactions
Antacids (containing aluminum or magnesium)
May reduce the absorption of Nerfid if taken concurrently. Administer Nerfid at least 2 hours after antacids.
CNS depressants (e.g., opioids, benzodiazepines)
May enhance the sedative effects of Nerfid, leading to increased drowsiness and dizziness.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include severe drowsiness, dizziness, ataxia, lethargy, and mild diarrhea. Management is primarily supportive; gastric lavage may be considered if recent. Hemodialysis may be effective in removing Nerfid from the system due to its renal excretion.
Pregnancy & Lactation
Pregnancy Category C: Animal studies have shown an adverse effect on the fetus, and there are no adequate and well-controlled studies in humans. It should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Nerfid is excreted in breast milk; a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.

Lab Monitoring
- Renal function tests (e.g., serum creatinine, creatinine clearance) should be monitored periodically in patients with renal impairment.
- Liver function tests in patients with pre-existing hepatic conditions.
Doctor Notes
- Monitor patients for signs of CNS depression and suicidal ideation, particularly during initiation of therapy or dose adjustments.
- Assess renal function before and during treatment, especially in elderly patients or those with pre-existing renal impairment.
- Educate patients on the importance of gradual dose tapering to avoid withdrawal symptoms.
Patient Guidelines
- Do not stop taking Nerfid abruptly, as this may lead to withdrawal symptoms. Follow your doctor's instructions for tapering the dose.
- Inform your doctor about any new or worsening side effects.
