Description
Overview of the medicine
Neugalin (Pregabalin) is an antiepileptic drug also used to treat neuropathic pain, fibromyalgia, and generalized anxiety disorder. It works by affecting certain chemicals in the brain that send pain signals across the nervous system.
Uses & Indications
Dosage
Adults
Initial dose usually 75 mg twice daily or 50 mg three times daily, adjusted based on response and tolerability, up to a maximum of 300 mg twice daily or 200 mg three times daily (total 600 mg/day).
Elderly
Lower initial doses and slower titration may be required due to potential decreased renal function.
Renal_impairment
Dosage adjustment is necessary for patients with impaired renal function based on creatinine clearance.
How to Take
Take orally with or without food. Swallow the capsule whole with water. Do not chew or crush.
Mechanism of Action
Pregabalin binds with high affinity to the alpha2-delta subunit of voltage-gated calcium channels in the central nervous system, which modulates the release of excitatory neurotransmitters like glutamate, noradrenaline, and substance P. This action is believed to be responsible for its anticonvulsant, analgesic, and anxiolytic effects.
Pharmacokinetics
Onset
Analgesic effects may be seen within 1 week for neuropathic pain. Anxiolytic effects may be noted within 1 week.
Excretion
Primarily excreted unchanged by the kidneys.
Half life
Approximately 6.3 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations occur within 1.5 hours.
Metabolism
Negligibly metabolized in humans; approximately 98% is excreted unchanged in the urine.
Side Effects
Contraindications
- Hypersensitivity to pregabalin or any component of the formulation.
- Severe allergic reactions to previous doses.
Drug Interactions
CNS depressants (opioids, benzodiazepines, alcohol)
May potentiate the sedative and cognitive effects of pregabalin.
Thiazolidinedione antidiabetic agents (e.g., rosiglitazone)
May increase the risk of peripheral edema and weight gain.
Storage
Store below 30°C, protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, confusion, agitation, and restlessness. Management is supportive and symptomatic; hemodialysis may be effective.
Pregnancy & Lactation
Pregnancy Category C: Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Excreted in breast milk; caution should be exercised when administered to a nursing mother.
